两种内固定方式治疗胫骨干骨折的疗效比较

摘　　要：目的比较桥接组合式内固定系统与交锁髓内钉固定治疗胫骨干骨折的效果。方法将79例胫骨干骨折患者根据内固定方式不同分对照组(采用交锁髓内钉固定治疗,40例)和观察组(采用桥接组合式内固定系统治疗,39例)。记录手术时间、术中出血量、切口感染发生率及骨折愈合时间,采用Johner-Wruhs评分标准评估患肢功能。结果患者均获得随访,时间12~35个月。两组手术时间、术中出血量、切口感染发生率、骨折愈合时间、患肢功能优良率比较差异均无统计学意义(P>0.05)。结论桥接组合式内固定系统治疗胫骨干骨折可获得与交锁髓内钉固定相近的临床效果,可根据患者软组织条件、骨折特性以及术者经验选择合适的内固定方式。Objective To compare the efficacy of bridging combined internal fixation system and interlocking intramedullary nailing fixation in treament of tibial shaft fracture.Methods The 79 patients with tibial shaft fractures were divided into control group(40 cases were adopted with interlocking intramedullary nailing fixation)and observation group(39 cases were treated with interlocking bridging combined internal fixation system)according to different internal fixation.The operation time,intraoperative blood loss,incidence of the wound infection,fracture healing time were recorded,and affected limb function was evaluated by using Johner-Wruhs score criterion.Results All patients were followed up for 12~35 months.There were no statistical differences in operation time,intraoperative blood loss,wound infection incidence rate,fracture healing time and the excellent-good rate of the affected limb function(P>0.05).Conclusions In the treatment of tibial shaft fractures,the bridging combined internal fixation system can achieve similar clinical effects to interlocking intramedullary nailing fixation.The surgeon can choose the appropriate internal fixation method according to the soft tissue conditions,fracture characteristics and personal experience.
