摘  要:目的探讨恶性肿瘤患者发生血流感染情况后病原菌的分布情况及耐药性。方法以2019年1月—2023年12月射阳县人民医院收诊恶性肿瘤患者106例作为研究对象进行回顾性分析,患者均出现血流感染问题,对其进行血培养,鉴定病原菌分布情况,同时进行药敏试验,探讨耐药性。结果在106例恶性肿瘤血流感染患者中,共分离出160株病原菌,涵盖了革兰氏阴性菌和阳性菌。106例恶性肿瘤血流感染患者的年龄为38~84岁。其中,以罹患肺癌和肝癌的患者数量居多。药敏试验结果显示,革兰氏阴性菌普遍对氨苄西林表现出较高的耐药性;革兰氏阳性菌对红霉素、苯唑西林表现出较高的耐药性。结论在恶性肿瘤患者中,血流感染的病原菌主要为革兰氏阴性菌,并且这些病原菌对各类抗菌药物的耐药性表现出显著差异。因此,临床治疗时应依据药敏试验结果,对病原菌的耐药性进行细致分析,并据此合理选用治疗药物,以期提升治疗效果。Objective To explore the distribution and drug resistance of pathogenic bacteria of bloodstream infections of patients with malignant tumors.Methods The paper reviewed and analyzed 106 malignant tumor patients in Sheyang County People's Hospital from January 2019 to December 2023 as research subjects,all patients had bloodstream infections,distribution of pathogens was identified with blood cultures.Drug resistance was explored based on drug sensitivity tests.Results Out of 106 malignant tumor cases with bloodstream infections,160 pathogenic bacterial strains were isolated,including Gram negative and Gram positive bacteria.Among them,Pseudomonas aeruginosa and Escherichia coli had a relatively high proportion.On the other hand,there were various species of Gram positive bacteria,including Staphylococcus aureus,Enterococcus faecalis,Enterococcus faecalis and coagulase negative Staphylococcus(including Staphylococcus epidermidis).Among them,proportion of Enterococcus faecalis and Staphylococcus aureus was significant.Drug sensitivity testing was carried out for bacterial genera with high proportion further.Gram negative bacteria generally had high resistance rate to ampicillin and low resistance rate to amikacin and meropenem.Gram positive bacteria had lower resistance rates to vancomycin and rifampicin,and higher resistance rate to erythromycin and benzylpenicillin.Conclusion Gram negative bacteria are main pathogens for bloodstream infections of patients with malignant tumors,there is difference in antibiotic resistance.Clinically,it is necessary to analyze pathogen resistance based on drug sensitivity results and choose treatment drugs reasonably to improve curative effect.
