经皮血管腔内成形术治疗自体动静脉内瘘狭窄或闭塞的疗效及术后内瘘失功的分析

摘　　要：目的分析对自体动静脉内瘘(AVF)狭窄或闭塞患者应用经皮血管腔内成形术(PTA)治疗的临床效果,以及术后患者内瘘失功情况。方法选取2023年11月—2024年10月本院收治的自体动静脉内瘘狭窄或闭塞患者100例,依据随机双盲分组法分为对照组(30例)和观察组(70例)。其中,对照组应用开放手术重建术式治疗,观察组应用PTA治疗。对比分析两组围手术期情况(包括手术时间、平均手术费、住院时间)、内瘘初级通畅率与平均通畅时间及并发症发生情况。结果观察组手术时间和住院时间均显著短于对照组,且平均手术费则高于对照组(P<0.05);两组内瘘初级通畅率、内瘘平均通畅时间差异显著,有可比性(P<0.05);较对照并发症总发生率而言,观察组明显更低(P<0.05)。结论PTA治疗AVF狭窄或闭塞,可有效缩短患者手术、住院时间和减少治疗费用,能够较好改善患者内瘘初级通畅率与平均通畅时间,减少并发症发生,值得临床推广。Objective To analyze clinical effect of percutaneous transluminal angioplasty(PTA)in treatment of stenosis or occlusion of autogenous arteriovenous fistula(AVF),and the postoperative fistula failure of patients.Methods The paper chose 100 patients with autogenous arteriovenous fistula stenosis or occlusion in our hospital from November 2023 to October 2024,and randomly divided them into the control group(30 cases)and the observation group(70 cases)with double-blind grouping method.The control group was treated with open surgical reconstruction,while the observation group with PTA.Perioperative conditions(including surgical time,average surgical cost,lenth of stay),primary patency rate and average patency time of internal fistula,and incidence of complications were compared and analyzed between two groups.Results The observation group had significantly shorter surgical time and length of stay than the control group,and higher average surgical cost than the control group(P<0.05).There was significant difference in the primary patency rate and average patency time between two groups,which was comparable(P<0.05).The observation group had significantly lower incidence of complications than the control group,(P<0.05).Conclusion PTA treatment can shorten surgery time and length of stay,reduce treatment costs,improve primary patency rate and average patency time,reduce complications incidence of patients with AVF stenosis or occlusion effectively,which is worthy of clinical promotion.
