摘  要:目的探讨肺结核呼吸衰竭患者的甲状腺激素水平变化及临床价值。方法回顾并分析2018年1月—2019年6月在广州市胸科医院确诊的120例肺结核患者与60例非肺结核患者的数据。针对肺结核患者,根据是否合并呼吸衰竭,将其分为肺结核合并呼吸衰竭组(n=60)和肺结核未合并呼吸衰竭组(n=60),同时选取60例非肺结核患者作为对照组。各组研究对象均是入院后采集血液,离心分离血清,检测血清总三碘甲状腺原氨酸(TT3)、总甲状腺素(TT4)、促甲状腺素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)水平。比较各组的检测结果,针对肺结核不同预后的检测结果进行对比。结果肺结核合并呼吸衰竭组、肺结核未合并呼吸衰竭组TT3、TT4、FT3指标水平均低于对照组(P<0.05),肺结核合并呼吸衰竭组TT3、TT4、FT3指标水平低于肺结核未合并呼吸衰竭组,组间差异有统计学意义(P<0.05)。三组患者TSH、FT4指标水平对比,组间差异无统计学意义(P>0.05)。肺结核呼吸衰竭患者死亡11例,死亡率18.33%,存活者TT3、TT4、FT3指标水平均高于死亡者,差异有统计学意义(P<0.05);存活者与死亡者TSH、FT4指标水平对比,组间差异无统计学意义(P>0.05)。结论检测血清甲状腺激素水平可以早期识别肺结核疾病,同时还可以判定患者肺结核发生情况,为疾病的预后也提供一定参考借鉴,值得推广。Objective To explore changes and clinical value of thyroid hormone levels of patients with pulmonary tuberculosis and respiratory failure.Methods The paper reviewed and analyzed 120 patients diagnosed with pulmonary tuberculosis and 60 non tuberculosis patients in ICU of Guangzhou Chest Hospital from January 2018 to June 2019,and divided them into two pulmonary tuberculosis and respiratory failure group(n=60),pulmonary tuberculosis and non respiratory failure group(n=60)and the control group of 60 case of non pulmonary tuberculosis based on complications of respiratory failure.Research subjects of each group were collected blood after admission,serum levels of total triiodothyronine(TT3),total thyroxine(TT4),thyroid stimulating hormone(TSH),free triiodothyronine(FT3)and free thyroxine(FT4)were detected after centrifugal separation.Detection indicators were compared between groups for different prognoses of pulmonary tuberculosis.Results Levels of TT3,TT4 and FT3 in tuberculosis and respiratory failure group and tuberculosis and non respiratory failure group were lower than the control group,difference was statistically significant between groups(P<0.05).Levels of TT3,TT4 and FT3 in the tuberculosis and respiratory failure group were lower than tuberculosis and non respiratory failure group,difference was statistically significant between groups(P<0.05).There was no statistically significant difference in TSH and FT4 levels among three groups(P>0.05).11 patients with pulmonary tuberculosis and respiratory failure died,with a mortality rate of 18.33%.Levels of serum TT3,TT4 and FT3 of survivors were higher than the deceased,difference was statistically significant(P<0.05).There was no statistically significant difference in serum TSH and FT4 levels between survivors and the deceased,(P>0.05).Conclusion Detection of serum thyroid hormone levels can identify pulmonary tuberculosis disease early,determine incidence of pulmonary tuberculosis of patients,and provide some certain reference for prognosis of the disease,wh
