多囊卵巢综合征所致不孕症患者血清INHB、AMH及性激素水平检测分析

摘　　要：目的分析多囊卵巢综合征(PCOS)所致不孕症患者血清抑制素B(INHB)、抗苗勒管激素(AMH)及性激素水平变化。方法选取2021年6月至2023年6月该院接诊的302例PCOS所致不孕症患者作为试验组,另选取同期在该院体检的300例月经周期正常的育龄期健康女性作为对照组。观察并比较两组血清INHB、AMH及性激素指标[血清黄体生成素(LH)、卵泡刺激素(FSH)、催乳激素(PRL)、孕酮(P)、睾酮(T)、雌二醇(E2)]及稳态模型胰岛素抵抗(HOMA-IR)水平变化。结果试验组血清AMH、INHB、HOMA-IR水平均明显高于对照组,差异有统计学意义(P<0.05)。试验组血清LH/FSH、PRL、T、P水平均明显高于对照组,血清E2水平明显低于对照组,差异有统计学意义(P<0.05)。随着治疗时间的推移,血清AMH、INHB、LH/FSH、PRL、T、P水平逐渐下降,E2水平逐渐升高(P<0.05)。AMH、INHB联合检测诊断PCOS所致不孕症的曲线下面积(AUC)为0.905,高于AMH、INHB单独检测的AUC(P<0.05)。血清AMH、INHB水平与FSH、LH、T、HOMA-IR、PRL、P均呈正相关(P<0.05),与血清E2水平呈负相关(P<0.05)。结论PCOS所致不孕症患者血清AMH、INHB水平明显上升,而且AMH、INHB与性激素水平存在相关性,检测血清AMH、INHB及性激素水平有助于PCOS所致不孕症的诊断。Objective To analyze the changes of serum inhibin B(INHB),anti-Müllerian hormone(AMH)and sex hormone levels in patients with infertility caused by polycystic ovary syndrome(PCOS).Methods A total of 302 patients with infertility caused by PCOS admitted to the hospital from June 2021 to June 2023 were selected as the experimental group,and 300 healthy women of childbearing age with normal menstrual cycle were selected as the control group.The levels of serum INHB,AMH and sex hormone indexes[serum luteinizing hormone(LH),follicle stimulating hormone(FSH),prolactin(PRL),progesterone(P),testosterone(T),estradiol(E2)]and homeostasis model assessment of insulin resistance(HOMA-IR)were observed and compared between the two groups.Results The serum levels of AMH,INHB and HOMA-IR in test group were significantly higher than those in control group,with statistical significance(P<0.05).Serum levels of LH/FSH,PRL,T and P in experimental groups were significantly higher than those in control group,and serum E2 levels were significantly lower than those in control group,with statistical significance(P<0.05).With the passage of treatment time,serum AMH,INHB,LH/FSH,PRL,T and P levels gradually decreased,and E2 levels gradually increased(P<0.05).The area under the curve(AUC)of AMH and INHB combined detection in the diagnosis of infertility caused by PCOS was 0.905,which was higher than the AUC of AMH or INHB detection alone(P<0.05).Serum AMH and INHB levels were positively correlated with FSH,LH,T and HOMA-IR,PRL,P(P<0.05),and negatively correlated with serum E2 levels(P<0.05).Conclusion The levels of AMH and INHB in serum of infertility patients caused by PCOS are significantly increased,and AMH and INHB are correlated with sex hormone levels.Detection of serum AMH,INHB and sex hormone levels is helpful for the diagnosis of infertility caused by PCOS.
