甲巯咪唑治疗甲状腺功能亢进症所致不良反应214例分析

摘　　要：目的分析甲巯咪唑治疗甲状腺功能亢进症的药品不良反应(ADR),为临床用药提供参考。方法采用回顾性研究法收集2021年4月至2023年2月在川北医学院附属三台医院采用甲巯咪唑治疗的甲状腺功能亢进症患者的病历资料,记录并统计在治疗期间应用甲巯咪唑出现的ADR,探讨ADR的严重程度及处理措施。结果930例采用甲巯咪唑治疗的甲状腺功能亢进症患者,发生ADR患者214例,未发生ADR患者716例,ADR发生率为23.01%。214例甲状腺功能亢进症患者中,男性48例(22.43%),女性166例(77.57%),女性患者数量高于男性患者(χ^(2)=130.131,P<0.05);年龄主要集中在10~20岁(26.64%)、21~35岁(36.45%)、36~50岁(20.09%)。10~50岁患者数量(83.18%)高于10岁以下及50岁以上的患者(16.82%)(χ^(2)=188.450,P<0.05)。214例甲状腺功能亢进症患者共发生ADR 424例次,主要为皮肤反应139例次(32.78%)、肌肉骨骼系统疾病103例次(24.29%)、胃肠系统反应97例次(22.88%)。结论川北医学院附属三台医院甲状腺功能亢进症患者年龄在10~50岁且多发生于女性人群,最常见的ADR为皮肤、骨骼肌肉及胃肠道反应,偶尔会出现粒细胞缺乏、肝损害等比较严重的ADR,临床需结合患者的个体情况采取相应的治疗措施。Objective To analyze the adverse reactions(ADRs)of methimazole in the treatment of hyperthyroidism and provide reference for clinical medication.Methods A retrospective study method was used to collect data on patients with hyperthyroidism treated with methimazole in Santai Hospital Affiliated to North Sichuan Medical College from April 2021 to February 2023,and the use of methimazole during treatment was recorded and counted.The severity and counter-measures of the adverse reactions were discussed.Results In 930 patients with hyperthyroidism treated with methimazole,214 patients developed ADR and 716 did not,and the incidence of ADR was 23.01%.Among the 214 patients with ADRs,48 were male(22.43%)and 166 female(77.57%).The number of female patients was higher than that of male ones(χ^(2)=130.131,P<0.05).The age was mainly between 10 and 20(26.64%),21-35(36.45%),36-50(20.09%).The percentage of patients between 10 and 50(83.18%)was higher than that of patients under 10 years old and over 50 years old(16.82%)(χ^(2)=188.450,P<0.05).A total of 424 ADRs occurred in the 214 patients and they were mainly skin reactions in 139 cases(32.78%),musculoskeletal system reactions in 103(24.29%)and gastrointestinal system reactions in 97(22.88%).Conclusion Hyperthyroidism patients treated in Santai Hospital Affiliated to North Sichuan Medical College are generally aged between 10 and 50 and the ADRs mostly occur in females.The most common ADRs are found in skin,skeletal muscle and gastrointestinal system.Occasionally,serious ADRs such as granulocyte deficiency and liver damage may occur,and corresponding treatment measures should be taken based on the patient’s condition.
