卡度尼利单抗致肾上腺皮质功能减退1例分析

摘　　要：目的分析卡度尼利单抗致肾上腺皮质功能减退的临床特点、危险因素及治疗等内容,为临床用药提供参考。方法对1例卡度尼利单抗致3级肾上腺皮质功能减退的病例进行分析,并对相关文献进行归纳总结。结果患者使用卡度尼利单抗后出现的乏力、恶心、低血钠等症状很可能为免疫相关性肾上腺皮质功能减退,患者停用卡度尼利单抗,并加用口服激素治疗后,不适症状消失,且依从性良好。结论卡度尼利单抗治疗过程中需关注患者的临床症状,如出现乏力、恶心、低钠血症等情况,应警惕药物相关肾上腺皮质功能减退的可能性,及时采取应对措施,对改善患者预后具有重要意义。Objective To analyze the clinical characteristics,risk factors,and treatment of adrenal insufficiency caused by cadonilimab and to explore the role of clinical pharmacists in the management of immune-related adverse events.Methods A case of 3rd grade adrenal insufficiency caused by cadonilimab was analyzed,and the related literature was summarized.Clinical pharmacists gave advice on management of adverse reactions,pharmaceutical care and medications.Results The fatigue,nausea,and hyponatremia experienced by the patient after receiving cadonilimab were likely attributed to immune-related adrenocortical insufficiency.After discontinuation of cadonilimab and start of oral steroid therapy,the patient had no more discomfort with good compliance.Conclusion During the treatment with cadonilimab,it is important to monitor the patient’s clinical symptoms,such as fatigue,nausea,hyponatremia,which may indicate the chance of drugrelated adrenal insufficiency.Quick countermeasures can be crucial to improving the patient’s prognosis.
