摘　　要：目的 观察脑利钠肽前体(pro-BNP)、肌酸激酶同工酶(CK-MB)、肌红蛋白(Mb)、心肌肌钙蛋白I(cTnI)、血同型半胱氨酸(Hcy)联合检验在心肌损伤疾病中的诊断价值。方法 选取心肌损伤疾病患者40例纳入此次研究并设为观察组,另选取同期进行健康体检40例健康人员设为对照组。两组均行pro-BNP、CK-MB、Mb、cTnI、Hcy检验。比较两组相关指标水平,各项指标单项检验及联合检验检出心肌损伤疾病的阳性检出情况及诊断效能。结果 观察组pro-BNP(853.75±310.16)pg/ml、CK-MB(31.90±3.53)ng/ml、Mb(99.42±8.16)μg/L、cTnI(1.92±0.60)ng/ml、Hcy(30.72±3.39)μmol/L较对照组的(201.63±90.24)pg/ml、(3.24±1.07)ng/ml、(57.18±6.92)μg/L、(0.04±0.01)ng/ml、(8.54±1.21)μmol/L更高(P<0.05)。pro-BNP、CK-MB、Mb、cTnI、Hcy联合检验对心肌损伤疾病的诊断灵敏度90.00%、准确度91.25%显著高于pro-BNP的47.50%、66.25%,CK-MB的50.00%、66.25%,Mb的40.00%、61.25%,cTnI的45.00%、62.50%,Hcy的52.50%、67.50%(P<0.05)。pro-BNP、CK-MB、Mb、cTnI、Hcy联合检验对心肌损伤疾病的诊断特异度与各项指标单项检验比较,无明显差异(P>0.05)。结论 pro-BNP、CK-MB、Mb、cTnI、Hcy联合检验在心肌损伤疾病中具有较为理想的诊断价值,其能够提高心肌损伤疾病阳性检出率、灵敏度、准确度,值得应用。Objective To observe the diagnostic value of combined test of brain natriuretic peptide precursor(pro-BNP),creatine kinase isoenzyme(CK-MB),myoglobin(Mb),cardiac troponin I(cTnI) and homocysteine(Hcy) in myocardial injury.Methods 40 patients with myocardial injury were included in this study and set as the observation group,and 40 healthy subjects who underwent physical examination during the same period were selected as the control group.Both groups were tested for pro-BNP,CK-MB,Mb,cTnI and Hcy,and the levels of relevant indexes,the positive detection of myocardial injury of single and combined test and the diagnostic efficacy were compared between the two groups.Results The observation group had pro-BNP of(853.75±310.16) pg/ml,CK-MB of(31.90±3.53) ng/ml,MB of(99.42±8.16) μg/L,cTnI of(1.92±0.60) ng/ml,and Hcy of(30.72±3.39) μmol/L,which were higher than(201.63±90.24) pg/ml,(3.24±1.07) ng/ml,(57.18±6.92) μg/L,(0.04±0.01) ng/ml,and(8.54±1.21) μmol/L in the control group(P<0.05).The sensitivity and accuracy of the combined test of pro-BNP,CK-MB,Mb,cTnI and Hcy were 90.00% and 91.25%,which were significantly higher than those of pro-BNP test(47.50% and 66.25%),CK-MB test(50.00% and 66.25%),Mb test(40.00% and 61.25%),cTnI test(45.00% and 62.50%) and Hcy test(52.50% and 67.50%)(P<0.05).There was no significant difference between the diagnostic specificity of combined test of pro-BNP,CK-MB,Mb,cTnI and Hcy and the single test of each index(P>0.05).Conclusion The combined test of pro-BNP,CK-MB,Mb,cTnI and Hcy has ideal diagnostic value in myocardial injury,which can improve the positive detection rate,sensitivity and accuracy of myocardial injury,and is worthy of application.
