蒲公英汤联合乳腺疏通治疗产后急性乳腺炎的疗效观察

摘　　要：目的 探讨分析蒲公英汤联合乳腺疏通治疗产后急性乳腺炎的疗效。方法 80 例产后急性乳腺炎患者,根据随机数表法分为头孢组和联合疏通组,每组 40 例。头孢组选择头孢克洛咀嚼片治疗,联合疏通组患者采用蒲公英汤联合乳腺疏通治疗。比较两组患者临床疗效、不良反应发生率以及治疗前后的临床症状缓解情况、血清炎症因子。结果 治疗 2 周后,联合疏通组治疗总有效率 95.00% 高于头孢组的 80.00%,差异有统计学意义 (P<0.05)。治疗 2 周后,联合疏通组患者的视觉模拟评分法 (VAS)评分 (0.74±0.17) 分、乳房肿块面积 (2.82±0.28)cm^(2)、局部皮肤发红范围 (4.92±1.15)cm^(2)均低于头孢组的 (1.22±0.22) 分、(4.29±0.59)cm^(2)、(6.44±1.37)cm^(2),差异有统计学意义 (P<0.05)。联合疏通组患者的血清白细胞介素 -6(IL-6)(8.96±2.41)pg/ml、C 反应蛋白 (CRP)(8.67±2.56)mg/L、降钙素原 (PCT)(0.53±0.21)ng/ml 均低于头孢组的 (14.25±3.67)pg/ml、(13.14±3.71)mg/L、(1.06±0.37)ng/ml,差异有统计学意义 (P<0.05)。联合疏通组不良反应发生率 12.50% 与头孢组的 22.50% 比较,差异无统计学意义 (P>0.05)。结论 蒲公英汤加乳腺疏通对产后急性乳腺炎有很好的效果,可以使患者的病情得到明显的改善,同时也可以降低患者的炎性因子水平。Objective To explore and analyze the curative effect of Pugongying Decoction combined with breast dredging on postpartum acute mastitis.Methods 80 patients with postpartum acute mastitis were divided into cephalosporin group and combined dredging group according to random number table method,with 40 cases in each group.The cephalosporin group was treated with cefaclor chewable tablets,and the combined dredging group was treated with Pugongying Decoction combined with breast dredging.The clinical efficacy,incidence of adverse reactions,relief of clinical symptoms and serum inflammatory factors before and after treatment were compared between the two groups.Results After 2 weeks of treatment,the total effective rate of 95.00%in combined dredging group was higher than 80.00%in cephalosporin group,and the difference was statistically significant (P<0.05).After 2 weeks of treatment,the visual analogue scale (VAS) score,breast lump and local skin redness range were (0.74±0.17) points,(2.82±0.28) cm^(2) and (4.92±1.15) cm^(2) in combined dredging group,which were lower than (1.22±0.22) min,(4.29±0.59) cm^(2) and (6.44±1.37) cm^(2) in cephalosporin group,and the differences were statistically significant (P<0.05).Combined dredging group had serum interleukin-6 (IL-6)of (8.96±2.41) pg/ml,C-reactive protein (CRP) of (8.67±2.56) mg/L,procalcitonin (PCT) of (0.53±0.21) ng/ml,serum IL-6 of (8.96±2.41) pg/ml,serum CRP of (8.67±2.56) mg/L,and serum PCT of (0.53±0.21) ng/ml,which were lower than (14.25±3.67) pg/ml,(13.14±3.71) mg/L,and (1.06±0.37) ng/ml in cephalosporin group,and the differences were statistically significant (P<0.05).There was no significant difference in the incidence of adverse reactions between the combined dredging group (12.50%) and the cephalosporin group (22.50%)(P>0.05).Conclusion Pugongying Decoction plus breast dredging has a good effect on postpartum acute mastitis,which can significantly improve the patient's condition while decreasing the level of inflammatory factors in patients.
