电针结合长针骶刺治疗糖尿病神经源性膀胱的疗效分析及尿流动力学研究

摘　　要：目的:探讨电针结合长针骶刺改善糖尿病神经源性膀胱患者临床症状及对尿流动力学影响。方法:将60例糖尿病神经源性膀胱患者随机分为药物组和针刺组各30例。药物组采用甲钴胺片和依帕司他片口服治疗,针刺组采用电针结合长针骶刺治疗,4周为一疗程,连续治疗两个疗程。观察治疗前后两组患者美国泌尿科协会症状指数评分、改良神经源性膀胱生活质量量表和尿流动力学指标最大尿流率、平均尿流率及膀胱残余尿量的变化。结果:治疗后针刺组总有效率高于药物组(P<0.05);两组患者治疗后AUA-SI评分、改良SF-Qualiveen评分、最大尿流率、平均尿流率及膀胱残余尿量与治疗前比较均有明显改善(P<0.01);治疗后针刺组患者AUA-SI评分、最大尿流率、平均尿流率及膀胱残余尿量变化优于药物组(P<0.05);治疗后改良SF-Qualiveen评分两组比较无统计学意义(P>0.05)。结论:电针结合长针骶刺治疗能显著改善糖尿病神经源性膀胱患者临床症状,改善尿流动力学相关指标。Objective:To explore the clinical efficacy of electro-acupuncture(EA)combined with long needle sacral acupuncture in treatment of diabetic neurogenic bladder(DNB)and its influence to urodynamics.Methods:60 patients with DNB were randomly divided into the medication group and the acupuncture group,with 30 patients in each group.The medication group was treated with oral Mecobalamine Tablet and Epalrestat Tablet,and the acupuncture group was treated with EA combined with long needle sacral acupuncture for two courses of eight weeks.The changes of American Urological Association symptom index(AUA-SI)score,modified Short-form Health Survey-Qualiveen(SF-Qualiveen)score,urodynamics indexes of MFR,AFR and RUV were observed before and after the treatment in the two groups,and the total effective rate was compared between the two groups.Results:The total effective rate of the acupuncture group was higher than that in the medication group(P<0.05).The scores of AUA-SI and modified SF-Qualiveen,the maximum urine flow rate,the average urine flow rate and the residual bladder urine volume were significantly improved after the treatment compared with those before the treatment in the two groups(P<0.01).The improvements in AUA-SI score,the maximum urine flow rate,the average urine flow rate and the residual bladder urine volume were more significant in the acupuncture group than those in the medication group(P<0.05).There was no statistical significance in the modified SF-Qualiveen score between the two groups after the treatment(P>0.05).Conclusion:EA combined with long needle sacral acupuncture can significantly improve the clinical symptoms and urodynamics indexes of patients with DNB and improve the life quality of the patients.
