化痰消结汤联合刮痧点刺法治疗小儿腺样体肥大临床研究

摘　　要：目的:观察化痰消结汤联合刮痧点刺法治疗小儿腺样体肥大的临床疗效。方法:选取2021年1月—2023年12月于河南中医药大学第一附属医院就诊的腺样体肥大患儿120例作为研究对象,按随机数字表法分为西药组、刮痧组、联合组各40例。西药组给予常规西药治疗,刮痧组在西药组的基础上联合刮痧点刺,联合组在刮痧组的基础上加服化痰消结汤。连续治疗4周后评估3组临床疗效,记录患儿主要症状消失时间及不良反应发生情况,比较3组治疗前后血清炎症因子[白细胞介素-4(IL-4)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]水平、腺样体肥大[腺样体和鼻咽腔比率(A/N值)]程度变化。结果:联合组总有效率为95.00%(38/40),分别高于刮痧组80.00%(32/40)、西药组72.50%(29/40),差异均有统计学意义(P<0.05);但刮痧组与西药组比较,差异无统计学意义(P>0.05)。联合组打鼾、鼻塞、张口呼吸、耳鸣等症状消失时间短于刮痧组、西药组(P<0.05),刮痧组上述各项症状消失时间短于西药组(P<0.05)。治疗后,3组血清IL-4、IL-6、TNF-α水平均较治疗前降低(P<0.05),且联合组血清IL-4、IL-6、TNF-α水平低于刮痧组、西药组(P<0.05),刮痧组IL-4、IL-6、TNF-α水平低于西药组(P<0.05)。治疗后,联合组A/N值均低于刮痧组、西药组(P<0.05),刮痧组A/N值低于西药组(P<0.05)。3组治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论:化痰消结汤联合刮痧点刺法治疗小儿腺样体肥大疗效显著,可在短时间内促进患儿主要症状缓解、减轻炎症反应及腺样体肥大。Objective:To observe the clinical effect of the therapy of Huatan Xiaojie Decoction combined with scraping and prickling method on adenoid hypertrophy in children.Methods:A total of 120 children with adenoidal hypertrophy who were treated in the First Affiliated Hospital of Henan University of Chinese Medicine from January 2021 to December 2023 were selected as the study objects and divided into the western medicine group,the scraping group and the combination group according to the random number table method,with 40 cases in each group.The western medicine group treated with conventional western medicine,the scraping group was additionally given scraping and prickling based on the treatment of the western medicine group,and the combination group was additionally treated with Huatan Xiaojie Decoction based on the treatment of the scraping group.After four weeks of continuous treatment,the clinical effects in the three groups were evaluated;the disappearance time of main symptoms and the incidence of adverse reactions were recorded;the levels of serum inflammatory factors[interleukin-4(IL-4),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)]and the degree of adenoid hypertrophy[adenoid-nasopharyngeal ratio(A/N ratio)]were compared before and after treatment among the three groups.Results:The total effective rate in the combination group was 95.00%(38/40),higher than that of 80.00%(32/40)in the scraping group and 72.50%(29/40)in the western medicine group,differences being significant(P<0.05);there was no significant difference being found in the comparison of clinical effect between the scraping group and the western medicine group(P>0.05).The disappearance time of snoring,nasal obstruction,mouth breathing,tinnitus and other symptoms in the combination group was respectively shorter than that in the scraping group and the western medicine group(P<0.05),and the disappearance time of the above symptoms in the scraping group was respectively shorter than that in the western medicine group(P<0.05).After treatmen
