摘  要:急性肾损伤(AKI)是一个全球性问题,影响着全球数百万患者,在过去几十年间,许多研究成果提升了对AKI的认识,2012年改善全球肾脏病预后组织(KDIGO)规范了AKI诊断标准并影响着后续临床治疗。谷氨酰胺(GLN)是一种非必需氨基酸,在健康人体中含量丰富,既往研究证明GLN补充剂可用于预防未感染患者的新感染或已经感染的危重患者的后续感染,是临床营养补充方案和推荐免疫抑制个体的一部分,随着AKI治疗指南的陆续更新及对GLN代谢的深入探索,越来越多研究者发现GLN在临床AKI及危重症患者中起到了不可忽视的保护作用,本篇综述介绍GLN代谢在AKI疾病发生的可能保护机制及临床引用,为更好地治疗AKI患者提供参考,从而有效保护AKI患者的肾功能,缓解疾病症状,改善疾病预后。Acute kidney injury(AKI)is a global problem,affecting millions of patients worldwide,and over the past few decades,many research results have improved our understanding of AKI,standardizing the diagnostic criteria for AKI and influencing subsequent clinical treatment in Kidney Diseases:Improving Global Outcomes(KDIGO)in 2012.Glutamine(GLN)is a non-essential amino acid that is abundant in healthy humans,and previous studies have shown that GLN supplementation can be used to prevent new infections in uninfected patients or subsequent infections in critically ill patients who have already been infected,and is part of the clinical nutritional supplement regimen and recommended immunosuppressive individuals.With the continuous updating of AKI treatment guidelines and the in-depth exploration of GLN metabolism,more and more researchers have found that GLN plays a non-ignorable protective role in clinical AKI and critically ill patients.This review introduces the possible protective mechanism and clinical reference of GLN metabolism in the occurrence of AKI diseases,so as to provide a reference for better treatment of AKI patients.It can effectively protect the renal function of AKI patients,alleviate the disease symptoms and improve the prognosis of the disease.
