尿微量白蛋白肌酐比、尿α1微球蛋白及尿β2微球蛋白在早期糖尿病肾病诊断中的效果评价

摘　　要：目的 探讨尿微量白蛋白肌酐比(albumin creatinine ratio, ACR)、α1微球蛋白(α1-microglobulin, α1-MG)及β2微球蛋白(β2-microglobulin, β2-MG)在早期糖尿病肾病(diabetic nephropathy, DN)中的诊断效果。方法 回顾性选取2023年6月-2024年8月商河县人民医院收治的80例DN患者的临床资料为探究组,并收集同期收治的80例单纯糖尿病患者的临床资料为对照组,所有患者均检测ACR、α1-MG、β2-MG的水平。比较两组ACR、α1-MG、β2-MG水平的差异,并评估各指标单独及联合检测的诊断效能。结果 探究组ACR、α1-MG、β2-MG水平均高于对照组,差异均有统计学意义(P均<0.05)。联合检测的灵敏度、特异度、准确度、阳性预测值、阴性预测值均高于ACR、α1-MG、β2-MG单一检测,差异均有统计学意义(P均<0.05)。结论 ACR、α1-MG、β2-MG检测在早期DN诊断中具有重要价值,且三者联合检测的诊断效能优于单一指标检测,有助于提高早期DN的诊断准确率,具有一定的临床应用价值。Objective To investigate the diagnostic effect of albumin creatinine ratio(ACR),α1-microglobulin(α1-MG)andβ2-microglobulin(β2-MG)in early diabetic nephropathy(DN).Methods The clinical data of 80 patients with DN admitted to Shanghe County People's Hospital from June 2023 to August 2024 were retrospectively selected as the study group,and the clinical data of 80 patients with simple diabetes admitted to the same period were collected as the control group.The levels of ACR,α1-MG andβ2-MG were detected in all patients.The differences of ACR,α1-MG andβ2-MG levels between the two groups were compared,and the diagnostic efficacy of single and combined of each index detection was evaluated.Results The levels of ACR,α1-MG andβ2-MG in the study group were higher than those in the control group,and the differences were statistically significant(all P<0.05).The sensitivity,specific-ity,accuracy,positive predictive value and negative predictive value of combined detection were higher than those of single detection of ACR,α1-MG andβ2-MG,and the differences were statistically significant(all P<0.05).Conclusion The detection of ACR,α1-MG andβ2-MG has important value in the diagnosis of early DN,and the di-agnostic efficacy of combined detection of the three is better than that of single index detection,which is helpful to im-prove the diagnostic accuracy of early DN and has certain clinical application value.
