糖皮质激素全身疗法与耳后注射疗法在2型糖尿病合并突发性耳聋患者中的疗效比较

摘　　要：目的 对比糖皮质激素全身疗法与耳后注射疗法在2型糖尿病合并突发性耳聋患者中的疗效。方法 回顾性选取2019年2月-2023年12月邳州市人民医院收治的57例2型糖尿病合并突发性耳聋患者的临床资料,按照治疗方法的不同分为全身组(28例,常规治疗+静脉滴注甲泼尼龙琥珀酸钠)和耳后组(29例,常规治疗+耳后注射甲泼尼龙琥珀酸钠),对比两组临床疗效、听阈水平以及血糖水平。结果 两组治疗总有效率比较,差异无统计学意义(P>0.05)。治疗后,两组听阈水平均优于治疗前,差异有统计学意义(P<0.05);但组间听阈水平对比,差异无统计学意义(P>0.05)。治疗后,两组血糖水平均较治疗前降低,且耳后组空腹血糖、餐后2 h血糖水平低于全身组,差异均有统计学意义(P均<0.05)。结论 糖皮质激素全身疗法与耳后注射疗法对2型糖尿病合并突发性耳聋的疗效相近,两种治疗方法均能改善听力,但耳后注射疗法在控制血糖方面优于糖皮质激素全身疗法。Objective To compare the efficacy of glucocorticoid systemic therapy and postauricular injection therapy in patients with type 2 diabetes mellitus complicated with sudden deafness.Methods The clinical data of 57 patients of type 2 diabetes mellitus complicated with sudden deafness admitted to the People's Hospital of Pizhou from Febru-ary 2019 to December 2023 were retrospectively selected.According to different treatment methods,they were divided into systemic group(28 cases,routine treatment+intravenous infusion of methylprednisolone sodium succinate)and post-auricular group(29 cases,routine treatment+post-auricular injection of methylprednisolone sodium succinate).The clinical efficacy,hearing threshold level and blood glucose levels of the two groups were compared.Results There was no significant difference in the total effective rate between the two groups(P>0.05).After treatment,the hearing threshold level of the two groups was better than that before treatment,and the difference was statistically significant(P<0.05).However,there was no significant difference in the level of hearing threshold between the two groups(P>0.05).After treatment,the blood glucose levels of the two groups were lower than those before treatment,and the lev-els of fasting blood glucose and 2-hour postprandial blood glucose in the post-auricular group were lower than those in the systemic group,the differences were statistically significant(both P<0.05).Conclusion The curative effect of glucocorticoid systemic therapy and post-auricular injection in the treatment of type 2 diabetes mellitus complicated with sudden deafness is similar.Both treatments can improve hearing,but post-auricular injection therapy is superior to glucocorticoid systemic therapy in controlling blood glucose.
