耳尖放血疗法对院前急诊血压急性升高的高血压患者的即时降压效果体会

摘　　要：目的观察对院前急诊血压急性升高的高血压患者使用耳尖放血疗法后的即时降压效果。方法选取急诊科院外出诊的血压急性升高的高血压患者120例,以抽签法1∶1进行分组,分为试验组(n=60)、对照组(n=60)。对照组给予常规治疗,试验组在常规治疗的基础上增加耳尖放血疗法。对比两组患者治疗前后不同时间点血压、治疗前后中医证候积分、治疗效果及不良反应发生情况。结果治疗后15 min、30 min、1 h,试验组收缩压分别为(161.52±11.33)、(152.71±9.65)、(139.52±9.82)mm Hg(1 mm Hg=0.133 kPa),舒张压分别为(92.11±10.31)、(89.63±9.72)、(81.62±9.44)mm Hg,均低于对照组的(170.62±12.58)、(161.33±11.95)、(147.69±9.66)mm Hg及(98.52±10.29)、(93.52±9.68)、(88.91±9.33)mm Hg(P<0.05)。治疗后,试验组眩晕、头痛、胸闷、烦躁积分分别为(1.68±0.66)、(1.66±0.58)、(1.57±0.49)、(1.49±0.48)分,低于对照组的(2.94±0.82)、(2.73±0.88)、(2.45±0.92)、(2.57±0.93)分(P<0.05)。试验组治疗总有效率100.00%高于对照组的93.33%(P<0.05)。试验组不良反应发生率5.00%低于对照组的16.67%(P<0.05)。结论院前急诊血压急性升高的高血压患者进行耳穴放血疗法治疗,即时降压效果更佳,可缩短患者救治时间,改善患者预后情况。Objective To observe the immediate antihypertensive effect of eartip bloodletting therapy in hypertensive patients with acute elevation of blood pressure in prehospital emergencies.Methods 120 cases of hypertensive patients with acute elevation of blood pressure in emergency department were selected and grouped into an experimental group(n=60)and a control group(n=60)at a ratio of 1∶1 by lottery method.The control group was given conventional treatment,and the experimental group added eartip bloodletting therapy on the basis of the conventional treatment.The blood pressure at different time points,traditional Chinese medicine syndrome score before and after treatment,therapeutic effect and occurrence of adverse reactions were compared between the two groups.Results At 15 min,30 min and 1 h after treatment,the systolic blood pressure of the experimental group were(161.52±11.33),(152.71±9.65)and(139.52±9.82)mm Hg(1 mm Hg=0.133 kPa),and the diastolic blood pressure were(92.11±10.31),(89.63±9.72)and(81.62±9.44)mm Hg,which were lower than(170.62±12.58),(161.33±11.95),(147.69±9.66)mm Hg and(98.52±10.29),(93.52±9.68),(88.91±9.33)mm Hg in the control group(P<0.05).After treatment,the scores of vertigo,headache,chest distress and irritability in the experimental group were(1.68±0.66),(1.66±0.58),(1.57±0.49)and(1.49±0.48)points,which were lower than(2.94±0.82),(2.73±0.88),(2.45±0.92)and(2.57±0.93)points in the control group(P<0.05).The total effective rate of 100.00%in the experimental group was higher than 93.33%in the control group(P<0.05).The incidence of adverse reactions of 5.00%in the experimental group was lower than 16.67%in the control group(P<0.05).Conclusion Eartip bloodletting therapy shows good immediate antihypertensive effect for hypertensive patients with acute elevation of blood pressure in prehospital emergencies,which can shorten the patient's rescue time and improve the patient's prognosis.
