通督调神针联合舌针治疗中风后吞咽障碍的临床疗效研究

摘　　要：目的分析通督调神针联合舌针治疗中风后吞咽障碍的临床效果。方法110例中风后吞咽障碍患者,根据治疗方式不同分为对照组与观察组,各55例。两组均接受吞咽康复训练,对照组给予常规针刺法治疗,观察组给予通督调神针联合舌针治疗。比较两组洼田饮水实验、标准吞咽功能评价量表(SSA)评分、吞咽造影检查(VFSS)评分、血清神经营养因子[血清神经营养因子(NTF)、脑源性神经营养因子(BDNF)、神经生长因子(NGF)]。结果治疗前,两组洼田饮水实验分级比较无差异(P>0.05);治疗后,观察组洼田饮水实验分级优于对照组(P<0.05)。治疗后,两组SSA、VFSS评分均优于本组治疗前,且观察组VFSS评分(8.53±1.67)分高于对照组的(7.12±1.20)分,SSA评分(18.26±2.34)分低于对照组的(19.89±3.07)分(P<0.05)。治疗后,两组NTF、BDNF、NGF水平均高于本组治疗前,且观察组NTF、BDNF、NGF水平分别为(5.85±1.21)、(22.89±2.78)、(88.18±12.45)ng/ml,均高于对照组的(4.63±0.83)、(20.55±2.22)、(72.57±10.80)ng/ml(P<0.05)。结论中风后吞咽障碍通过通督调神针联合舌针治疗,患者吞咽功能有效改善,血清神经营养因子水平升高。Objective To analyze the clinical effect of Tongdu Tiaoshen acupuncture combined with tongue acupuncture in the treatment of post-stroke swallowing disorder.Methods 110 patients with dysphagia after stroke were divided into control group and observation group according to different treatment modes,with 55 cases in each group.Both groups received swallowing rehabilitation training.On this basis,the control group was given routine acupuncture treatment,and the observation group was given Tongdu Tiaoshen acupuncture combined with tongue acupuncture treatment.Both groups were compared in terms of water swallow test,standardized swallowing assessment(SSA)score,videofluoroscopic swallowing study(VFSS)score,serum neurotrophic factor[neurotrophic factor(NTF),brain-derived neurotrophic factor(BDNF)and nerve growth factor(NGF)].Results Before treatment,there was no difference in water swallow test grading between the two groups(P>0.05).After treatment,the water swallow test grading in the observation group was better than that in the control group(P<0.05).After treatment,SSA and VFSS scores in both groups were better than those before treatment;the observation group had higher VFSS score of(8.53±1.67)points than(7.12±1.20)points in the control group,and lower SSA score of(18.26±2.34)points than(19.89±3.07)points in the control group(P<0.05).After treatment,the levels of NTF,BDNF and NGF in both groups were higher than those before treatment;the levels of NTF,BDNF and NGF in the observation group were(5.85±1.21),(22.89±2.78)and(88.18±12.45)ng/ml,which were higher than(4.63±0.83),(20.55±2.22)and(72.57±10.80)ng/ml in the control group(P<0.05).Conclusion Tongdu Tiaoshen acupuncture combined with tongue acupuncture can effectively improve the swallowing function and increase the level of serum neurotrophic factor in patients with post-stroke swallowing disorder.
