摘　　要：目的 分析糖尿病足(diabetic foot, DF)患者就诊延迟现状,探讨其影响因素,为DF患者及高危人群相关干预方案的拟定提供依据。方法 采取目的抽样法,回顾性选取2022年2月—2023年12月德阳市人民医院医院住院及门诊就诊的232例DF患者的临床资料,采用DF患者社会人口学资料调查问卷、临床资料调查表、就诊延迟相关问卷等进行问卷调查,根据就诊时间分为未延迟组(<24 h)和延迟组(≥24 h),采用多因素Logistic回归分析DF患者就诊延迟的影响因素。结果 单因素分析显示,两组患者年龄、文化程度、家庭人均年收入、是否有经济负担、接受DF健康指导、了解DF危险信号、自我管理行为量表得分、足部溃疡面积、门诊随访频次及是否留在家中自行处理或观察比较,差异均有统计学意义(P均<0.05)。多因素Logistic回归分析显示,文化程度为小学及以下、有经济负担、留在家中自行处理或观察为DF患者就诊延迟的影响因素(P均<0.05)。结论 DF患者就诊延迟现象显著且受多种因素左右,医务人员应根据不同影响因素对患者进行个体化健康指导和行为干预,减少因就诊延迟造成的截肢等严重后果,改善DF预后。Objective To analyze the current situation of treatment delay in patients with diabetic foot(DF),and to ex-plore its influencing factors,so as to provide a basis for the formulation of relevant intervention programs for DF pa-tients and high-risk groups.Methods The clinical data of 232 patients with DF who were hospitalized and outpatients in Deyang People's Hospital from February 2022 to December 2023 were selected as the research objects by purpo-sive sampling method.The social demographic data questionnaire,clinical data questionnaire and delay-related ques-tionnaire of DF patients were used for questionnaire survey.According to the time of treatment,they were divided into non-delay group(<24 h)and delay group(≥24 h).Multivariate Logistic regression analysis was used to analyze the in-fluencing factors of delay in treatment of DF patients.Results Univariate analysis showed that there were significant differences between the two groups in age,education level,family income per capita,economic burden,receiving DF health guidance,understanding DF risk signals,Self-management Behavior Scale score,foot ulcer area,outpatient follow-up frequency and whether to stay at home for self-treatment or observation(all P<0.05).Multivariate Logistic regression analysis showed that education level was primary school or below,economic burden,staying at home and observing the delay of treatment in patients with DF were the influencing factors(all P<0.05).Conclusion The delay of treatment in DF patients is significant and affected by many factors.Medical staff should provide individualized health guidance and behavioral intervention to patients according to different influencing factors,reduce the serious consequences such as amputation caused by delayed treatment,and improve the prognosis of DF.
