摘　　要：目的 探讨胱抑素C(cystatin C, CysC)、血清肌酐(serum creatinine, Scr)、血清尿素(serum urea, Sur)联合尿微量白蛋白(microalbuminuria, mAlb)检测在糖尿病肾病(diabetic nephropathy, DN)诊断中的灵敏度和特异度。方法 回顾性选取2023年1月-2024年5月济南市济阳区中医医院收治的67例DN患者的临床资料为探究组,并收集同期82例单纯糖尿病患者的临床资料为对照组。比较两组CysC、Scr、Sur及mAlb水平,并评估上述指标单独及联合检测的诊断效能。结果 探究组CysC、Scr、Sur、mAlb水平均高于对照组,差异均有统计学意义(P均<0.05)。联合检测的灵敏度、特异度、准确度、阳性预测值及阴性预测值均高于单一指标检测,差异均有统计学意义(P均<0.05)。结论 CysC、Scr、Sur联合mAlb检测在DN诊断中具有较高的灵敏度和特异度,其诊断效能优于单一指标检测,可为DN的早期诊断提供有力支持。Objective To investigate the sensitivity and specificity of cystatin C(CysC),serum creatinine(Scr),serum urea(Sur)combined with microalbuminuria(mAlb)in the diagnosis of diabetic nephropathy(DN).Methods The clini-cal data of 67 patients with DN admitted to Jinan Jiyang District Hospital of Traditional Chinese Medicine from Janu-ary 2023 to May 2024 were retrospectively selected as the study group,and the clinical data of 82 patients with simple diabetes were collected as the control group.The levels of CysC,Scr,Sur and mAlb were compared between the two groups,and the diagnostic efficacy of the above indicators alone and in combination was evaluated.Results The levels of CysC,Scr,Sur and mAlb in the study group were higher than those in the control group,and the differences were statistically significant(all P<0.05).The sensitivity,specificity,accuracy,positive predictive value and negative pre-dictive value of combined detection were higher than those of single index detection,and the differences were statisti-cally significant(all P<0.05).Conclusion The detection of CysC,Scr,Sur combined with mAlb has high sensitivity and specificity in the diagnosis of DN,and its diagnostic efficiency is better than that of single index detection,which can provide strong support for the early diagnosis of DN.
