加味黄芪桂枝五物汤治疗糖尿病周围神经病变对患者肾功能、下肢神经传导速度的影响分析

摘　　要：目的 分析加味黄芪桂枝五物汤治疗糖尿病周围神经病变对患者肾功能、下肢神经传导速度的影响。方法 选取2021年7月-2024年6月保定市满城区人民医院中医科收治的132例糖尿病周围神经病变患者为研究对象,根据不同的治疗方法分为两组,各66例。对照组采用依帕司他与甲钴胺治疗,研究组在对照组基础上采用加味黄芪桂枝五物汤治疗,比较两组体征指标、神经传导速度、肾功能指标及中医证候积分。结果 治疗后,研究组空腹血糖、血尿素氮、总胆固醇水平均低于对照组,差异均有统计学意义(P均<0.05)。治疗后,研究组正中神经运动传导速度、正中神经感觉传导速度、腓总神经运动传导速度、腓总神经感觉传导速度均高于对照组,差异均有统计学意义(P均<0.05)。治疗后,研究组24 h尿蛋白定量、血白蛋白、β2-微球蛋白、血肌酐水平均低于对照组,差异均有统计学意义(P均<0.05)。治疗后,研究组水肿、口干舌燥、腰腿酸软、肢体麻木评分均低于对照组,差异均有统计学意义(P均<0.05)。结论 加味黄芪桂枝五物汤用于治疗糖尿病周围神经病变可提高神经传导速度,降低血糖、血尿素氮、总胆固醇水平,促进肾功能恢复。Objective To analyze the effect of modified Huangqi Guizhi Wuwu Decoction on renal function and lower limb nerve conduction velocity in patients with diabetic peripheral neuropathy.Methods A total of 132 patients with diabetic peripheral neuropathy admitted to the Department of Traditional Chinese Medicine of Mancheng District People's Hospital of Baoding from July 2021 to June 2024 were selected as the research objects.According to differ-ent treatment methods,they were divided into two groups,66 cases in each group.The control group was treated with epalrestat and mecobalamin,and the study group was treated with modified Huangqi Guizhi Wuwu Decoction on the basis of the control group.The physical indexes,nerve conduction velocity,renal function indexes and traditional Chi-nese medicine symptom scores of the two groups were compared.Results After treatment,the levels of fasting blood glucose,blood urea nitrogen and total cholesterol in the study group were less than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the median nerve motor conduction velocity,me-dian nerve sensory conduction velocity,common peroneal nerve motor conduction velocity and common peroneal nerve sensory conduction velocity in the study group were greater than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the levels of 24 h urine protein,serum albumin,β2-microglobu-lin and serum creatinine in the study group were less than those in the control group,and the differences were statisti-cally significant(all P<0.05).After treatment,the scores of edema,dry mouth and tongue,soreness and weakness of waist and legs,and numbness of limbs in the study group were less than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion Modified Huangqi Guizhi Wuwu Decoction can accelerate nerve conduction velocity,reduce blood glucose,blood urea nitrogen and total cholesterol levels,and promote re
