“通脑活络”针刺疗法配合中频脉冲治疗仪对脑卒中后偏瘫患者肢体运动功能及肌张力的影响

摘　　要：目的:观察“通脑活络”针刺疗法配合中频脉冲治疗仪对脑卒中后偏瘫患者肢体运动功能及肌张力的影响。方法:将67例脑卒中后偏瘫患者随机分为对照组33例和观察组34例,两组均给予常规药物治疗和康复锻炼,对照组在此基础上采用中频脉冲治疗仪治疗,观察组在对照组基础上采用“通脑活络”针刺疗法治疗。比较两组临床疗效、中医症候积分、肢体运动功能、平衡和步态分析结果、肌张力及不良反应。结果:治疗8周后,观察组总有效率高于对照组,差异具有统计学意义(P<0.05);观察组中医症候量化积分及总分低于对照组,差异具有统计学意义(P<0.05);观察组Wolf运动功能测试量表(WMFT)、Fugl-Meyer下肢部分量表(FMA-LE)和功能综合评定(FCA)量表评分高于对照组,差异具有统计学意义(P<0.05);观察组“起立-行走”计时测试(TUG)、10 m步行时间短于对照组,患侧/健侧单支撑相大于对照组,患侧步长长于对照组,差异具有统计学意义(P<0.05);观察组上肢、下肢改良Ashworth分级整体比较优于对照组,差异具有统计学意义(P<0.05)。结论:“通脑活络”针刺疗法配合中频脉冲治疗仪可以显著改善脑卒中后偏瘫患者肢体运动功能、平衡性及步态,降低肌张力,疗效优于单纯中频脉冲治疗仪治疗。Objective:To observe the effects of Tongnao Huoluo(activating Brain and dredging collateral)acupuncture combined with medium frequency pulse therapy on limb motor function and muscle tension in patients with post-stroke hemiplegia.Methods:67 patients with post-stroke hemiplegia were randomly divided into the control group(n=33)and the observation group(n=34).Both groups were given conventional medication and rehabilitation exercise;on which basis,the control group was treated with medium frequency pulse therapy,and the observation group was treated with Huonao Tongluo acupuncture on the treatment basis of the control group.Clinical efficacy,TCM symptom scores,limb motor function,balance and gait analysis results,muscle tension and adverse reactions were compared between the two groups after the treatment.Results:After 8 weeks of treatment,the total effective rate of the observation group was significantly higher than that of the control group(P<0.05).The scores of TCM symptoms and the total score were lower in the observation group than those in the control group(P<0.05).The scores of Wolf Motor Function Scale(WMFT),Fugl-Meyer Assessment-Lower Extremity(FMA-LE)and functional comprehensive assessment(FCA)were higher in the observation group than those in the control group(P<0.05).The timed up and go(TUG)and 10 m walking time were shorter in the observation group than those in the control group,the single support phase of the affected side/healthy side was bigger in the treatment group than that in the control group,and the step length of the affected side was longer in the observation group than that in the control group(P<0.05).The improved Ashworth grades of upper and lower limbs were better in the observation group than those in the control group(P<0.05).Conclusion:Tongnao Huoluo acupuncture combined with medium frequency pulse therapy can significantly improve the limb motor function,balance and gait of patients with post-stroke hemiplegia,it can also reduce the muscle tension,and the efficacy is better than t
