三经六穴温阳通络针法联合椎间孔镜下髓核摘除术治疗腰椎间盘突出症的临床研究

摘　　要：目的:探讨三经六穴温阳通络针法联合椎间孔镜下髓核摘除术治疗腰椎间盘突出症(LDH)的疗效。方法:选取来本院治疗的LDH患者102例,随机分为两组各51例,对照组给予椎间孔镜下髓核摘除术,观察组联合三经六穴温阳通络针法,治疗后对比两组疗效、JOA评分、生活质量评分、临床症状情况、VAS评分及血清炎性因子水平变化。结果:观察组总有效率明显较高,差异具有统计学意义(P<0.05);经过治疗,两组患者在临床症状评分以及JOA总分均呈现显著提升,差异具有统计学意义(P<0.05),且观察组的提升幅度更为显著,差异具有统计学意义(P<0.05)。同时,两组的生活质量评分也在治疗后显著提高,差异具有统计学意义(P<0.05),其中观察组的提升效果更为突出,差异具有统计学意义(P<0.05)。此外,直腿抬高试验的阳性率方面,治疗后两组患者出现明显下降,而腰痛、下肢放射痛评分和腰背肌后伸ROM方面则全部明显上升,差异具有统计学意义(P<0.05);血清炎性因子浓度水平方面,相较于治疗前,两组治疗后全部出现了下降情况(P<0.05),且观察组改善更为明显,差异具有统计学意义(P<0.05)。结论:采用三经六穴温阳通络针法联合椎间孔镜下髓核摘除术治疗LDH具有较好的疗效。Objective:To explore the therapeutic effect of Three-meridian and Six-point Warming Yang and Activating Collateral Acupuncture combined with transforaminal endoscopic nucleotomy in treatment of lumbar disc herniation(LDH).Methods:102 patients with LDH were randomly divided into the control group and the observation group,with 51 patients in each group.The control group was given transforaminal endoscopic nucleotomy;on which basis,the observation group was also treated with Three-meridian and Six-point Warming Yang and Activating Collateral Acupuncture.The therapeutic effect was compared between the two groups.The changes in JOA score,quality of life score,clinical symptoms,VAS score and serum levels of inflammatory factors were compared between the two groups after the treatment.Results:The total effective rate of the observation group was obvious higher than that of the control group(P<0.05).The clinical symptoms scores and the total score of JOA were significantly increased after the treatment compared with those before the treatment in the two groups(P<0.05),and the increases were more significant in the observation group than those in the control group(P<0.05).The scores of quality of life were obviously improved after the treatment compared with those before the treatment in the two groups(P<0.05),and the improvements were more significant in the observation group than those in the control group(P<0.05).The positive rate of the straight leg raise test was obviously decreased,whereas the scores of lower back/lower limb radiation pain and the ROM of lower back muscle extension were significantly increased after the treatment compared with those before the treatment in the two groups(P<0.05).The serum levels of inflammatory factors were significantly decreased after the treatment compared with those before the treatment in the two groups(P<0.05),and the decreases in the observation group were more significant than those in the control group(P<0.05).Conclusion:Three-meridian and Six-point Warming Yang and Activa
