摘  要:目的分析阴道灌洗液中IgG Fc段结合蛋白(FCGBP)在阴道微环境中的作用及其与高危人乳头瘤病毒(HR-HPV)持续感染风险的关系。方法选取唐山市妇幼保健院2021年3月至2023年1月的518例HR-HPV感染患者,检测其阴道灌洗液中FCGBP、白细胞介素(IL)水平及阴道微生态。随访12个月后复查HR-HPV。用Logistic回归分析HR-HPV持续感染的风险因素。用Spearman和多元线性回归分析FCGBP的相关影响因素。结果菌群密集度Ⅰ级(OR=3.880,95%CI:1.233~12.208)、pH>4.5(OR=5.036,95%CI:1.497~16.944)、高IL-1β(OR=2.249,95%CI:1.681~3.008)和高IL-2(OR=2.456,95%CI:1.676~3.598)水平是HR-HPV持续感染的独立危险因素,高FCGBP(OR=0.166,95%CI:0.093~0.296)水平是HR-HPV持续感染的独立保护因素(P<0.05)。FCGBP与菌群密集度、菌群多样性、pH、过氧化氢、唾液酸苷酶、IL-1β和IL-2呈负相关关系,与IL-8和IL-21呈正相关关系(r_(s)值分别为-0.290、-0.191、-0.232、-0.186、-0.154、-0.483、-0.223、0.403和0.436,P<0.05)。菌群密集度、IL-1β和IL-2与FCGBP独立负相关,IL-8和IL-21与FCGBP独立正相关(标准回归系数值分别为-0.076、-0.335、-0.163、0.256和0.428,P<0.05)。结论FCGBP水平高提示HR-HPV持续感染风险低。FCGBP水平与菌群密集度及炎症因子IL-1β、IL-2、IL-8和IL-21之间存在相关性。提示FCGBP可能通过与上述因子共同作用影响HR-HPV持续感染,但具体机制仍有待阐明。Objective To analyze the role of IgG Fc-binding protein(FCGBP)in vaginal lavage fluid within the vaginal microenvironment and its association with the risk of persistent high-risk human papillomavirus(HR-HPV)infection.Methods A total of 518 HR-HPV-infected patients treated at Tangshan Maternal and Child Health Hospital from March 2021 to January 2023 were selected.The levels of FCGBP,interleukin(IL)in their vaginal lavage fluid and vaginal microecology were assessed.HR-HPV was rechecked after a 12-month follow-up.Logistic regression analysis was conducted to identify the determining factors for persistent HR-HPV infection.Spearman correlation and multiple linear regression were used to identify factors associated with FCGBP.Results Grade I bacterial density(OR=3.880,95%CI:1.233-12.208),pH>4.5(OR=5.036,95%CI:1.497-16.944),high levels of IL-1β(OR=2.249,95%CI:1.681-3.008),and IL-2(OR=2.456,95%CI:1.676-3.598)were independent risk factors for persistent HR-HPV infection,and high FCGBP level(OR=0.166,95%CI:0.093-0.296)was identified as an independent protective factor for persistent HR-HPV infection(P<0.05).Bacterial density,diversity,pH,hydrogen peroxide,sialidase,IL-1β,and IL-2 were negatively correlated with FCGBP,while IL-8 and IL-21 were positively correlated(r_(s)=-0.290,-0.191,-0.232,-0.186,-0.154,-0.483,-0.223,0.403,and 0.436,respectively;P<0.05).Bacterial density,IL-1β,and IL-2 were independently and negatively associated with FCGBP,while IL-8 and IL-21 were independently and positively associated(standardized regression coefficients:-0.076,-0.335,-0.163,0.256,and 0.428,respectively;P<0.05).Conclusion High FCGBP levels indicate a reduced risk of persistent HR-HPV infection.FCGBP expression correlates with bacterial density and inflammatory cytokines,including IL-1β,IL-2,IL-8,and IL-21.These findings suggest that FCGBP may modulate HR-HPV persistence through interactions with these factors;however,the underlying mechanisms remain to be elucidated.
