POCT检测心肌酶谱对急性冠脉综合征的诊断价值分析

摘　　要：目的:探讨与分析床旁快速检测技术(Point-of-care testing,POCT)检测心肌酶谱对急性冠脉综合征(Acute coronary syndrome,ACS)的诊断价值。方法:在2023年1月到2024年9月时期将到我院进行诊治的急性冠脉综合征患者中筛选出82例,将其作为ACS组,同时在相同时间段选择来进行健康体检的82例正常人群,将其作为对照组。所有人群都给予超声检查,记录超声特征。同时所有人群都采用POCT检测心肌酶谱-血清肌酸激酶(Creatine kinase,CK)、肌酸激酶同工酶(Creatine kinase isoenzyme,CKMB)、乳酸脱氢酶(Lactate dehydrogenase,LDH)含量并进行相关性分析。结果:ACS组的左室射血分数显著低于对照组(P<0.05),而左心房内径、左心室内径、左室后壁厚度与对照组相比均提高(P<0.05)。ACS组的血清LDH、CKMB、CK含量与对照组相比均提高(P<0.05)。ACS组的血清心肌酶谱指标阳性率分别为81.71%、86.59%、86.59%,显著高于对照组的1.22%、2.44%、3.66%(P<0.05)。在ACS组中,Pearson分析显示血清LDH、CKMB、CK含量与左室射血分数、左心房内径、左心室内径、左室后壁厚度都呈显著相关性(P<0.05)。结论:POCT检测心肌酶谱对急性冠脉综合征的诊断具有很高的价值,也具有快速、便捷、准确等特征,值得在临床上推广使用。Objective:To explore and analyze the diagnostic value of point-of-care testing(POCT)for cardiac enzyme profiles in acute coronary syndrome(ACS).Methods:From January 2023 to September 2024,82 cases of patients diagnosed with acute coronary syndrome in our hospital were selected as the ACS group,and another 82 cases of individuals undergoing routine health check-ups were randomly selected as the control group.All subjects were underwent ultrasound examination,and the ultrasound characteristics were recorded.At the same time,all subjects were tested for cardiac enzyme profiles using POCT,included serum creatine kinase(CK),creatine kinase isoenzyme(CKMB),and lactate dehydrogenase(LDH),and correlation analysis was performed.Results:The left ventricular ejection fraction in the ACS group was significantly lower than that in the control group(P<0.05),while the left atrial diameter,left ventricular diameter,and left ventricular posterior wall thickness were statistically significantly increased compared to the control group(P<0.05).The serum levels of LDH,CKMB,and CK in the ACS group were statistically significantly higher than those in the control group(P<0.05).The positive rates of serum cardiac enzyme profile indicators in the ACS group were 81.71%,86.59%,and 86.59%,respectively,significantly higher than those in the control group at 1.22%,2.44%,and 3.66%(P<0.05).In the ACS group,Pearson analysis showed that the serum levels of LDH,CKMB,and CK were significantly correlated with the left ventricular ejection fraction,left atrial diameter,left ventricular diameter,and left ventricular posterior wall thickness(P<0.05).Conclusion:POCT for cardiac enzyme profiles has a high diagnostic value for acute coronary syndrome and is characterized by being fast,convenient,and accurate,and is worth promoting in clinical practice.
