摘 要:目的:探讨硫酸镁雾化吸入联合异丙托溴铵治疗小儿毛细支气管炎的效果及对气道阻力指标的影响。方法:选取2022年12月至2023年12月期间本院接收的78例小儿毛细支气管炎患儿作为研究对象。随机将患儿分为对照组和观察组,每组各39例。对照组使用异丙托溴铵治疗;观察组使用硫酸镁雾化吸入联合异丙托溴铵治疗。分析比较两组的疗效、症状消失时间、炎症因子[血清降钙素原(Procalcitonin,PCT)、C反应蛋白(C-reactive Protein,CRP)]水平、气道阻力指标[近端气道黏性阻力(Near end airway viscous resistance,R20)、气道总黏性阻力(Total airway viscous resistance,R5)、气道总阻抗(Total airway impedance,Z5)]以及不良反应。结果:察组的治疗有效率明显高于对照组(P<0.05)。观察组的咳嗽、憋喘、哮鸣音、湿啰音等各项临床症状消失时间均比对照组显著缩短(P<0.05)。治疗1 w后,两组的各项炎症因子水平均下降,且观察组的PCT、CRP水平均明显低于对照组(P<0.05)。治疗1 w后,两组的各项气道阻力指标水平均有降低,且观察组R20、R5、Z5均比对照组显著降低(P<0.05)。两组的不良反应发生率无显著差异。结论:异丙托溴铵联合硫酸镁雾化吸入治疗小儿毛细支气管炎,能提高疗效,减轻患者的临床症状、降低炎症因子水平,改善患儿气道阻力,且安全性较好。Objective:To investigate the effect of magnesium sulfate aerosol inhalation combined with ipratropium bromide in the treatment of children with bronchiolitis and its influence on airway resistance indexes.Methods:78 children with bronchiolitis admitted to our hospital from December 2022 to December 2023 were selected as the study objects.The children were divided into control group and observation group with 39 cases in each group randomly.The control group was treated with ipratropium bromide.Observation group was treated with atomized inhalation of magnesium sulfate combined with ipratropium bromide.The efficacy,symptom resolution time,levels of inflammatory factors[serum procalcitonin(PCT),C-reactive protein(CRP)],airway resistance indexes[proximal airway viscosity resistance(R20),total airway viscosity resistance(R5),total airway impedance(Z5)],and adverse reactions of the two groups were analyzed and compared.Results:The effective rate of the observation group was significantly higher than that of the control group(P<0.05).The disappearance time of cough,wheezing,wheezing and rales in observation group was significantly shorter than that in control group(P<0.05).After 1 week of treatment,the levels of various inflammatory factors in both groups decreased,and the levels of PCT and CRP in the observation group were significantly lower than those in the control group(P<0.05).After 1 week of treatment,the levels of airway resistance indexes in both groups were decreased,and R20,R5 and Z5 in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion:Ipratropium bromide combined with magnesium sulfate aerosol inhalation in the treatment of children with bronchiolitis can improve the efficacy,reduce the clinical symptoms of patients,reduce the level of inflammatory factors,improve the airway resistance of children with good safety.
