吸入用N-乙酰半胱氨酸联合糖皮质激素对肺炎患儿症状改善、炎性指标的影响

摘　　要：目的:分析肺炎患儿接受吸入用N-乙酰半胱氨酸(N-acetylcysteine,NAC)联合糖皮质激素治疗的优势。方法:将新乡市第一人民医院2021年1月至2022年12月收治的87例肺炎患儿根据随机数字表法分为对照组(43例,解热镇痛、止咳祛痰等+布地奈德)和观察组(44例,加用吸入用NAC)。比较两组各项指标变化情况。结果:治疗7 d后,观察组患儿总优良率高于对照组;观察组咳嗽和喘息消失、气促改善、住院时间短于对照组;治疗7 d后观察组潮气量(Tidal Volume,VT)、最大呼气流量(Peak Expiratory Flow,PEF)高于对照组,呼吸频率(Respiratory Rate,RR)、血清干扰素γ(Interferon-γ,IFN-γ)、白细胞介素-18(Interleukin-18,IL-18)、白细胞介素-17(Interleukin-17,IL-17)水平低于对照组(P<0.05)。结论:针对肺炎患儿,吸入用NAC联合糖皮质激素治疗能够提高临床疗效,改善患儿临床各项指标,整体安全性高。Objective:To analyze the advantages of inhaled N-acetylcysteine(NAC)combined with glucocorticoid therapy in children with pneumonia.Methods:A total of 87 children with pneumonia admitted to Xinxiang First People's Hospital from January 2021 to December 2022 were divided into the control group(43 cases,antipyretic analgesic,cough expectorant,etc+budesonide)and the observation group(44 cases,plus NAC for inhalation)according to the random number table method.The changes of each index of the two groups were compared.Results:After 7 days of treatment,the total excellent and good rate of the observation group was higher than that of the control group;the disappearance of cough and wheezing,improvement of shortness of breath and hospitalization time of the observation group were shorter than those of the control group;after 7 days of treatment,the levels of tidal volume(VT)and peak expiratory flow(PEF)of the observation group were higher than those of the control group,and the levels of respiratory rate(RR),serum interferon-γ(IFN-γ),interleukin-18(IL-18)and interleukin-17(IL-17)were lower than those of the control group(P<0.05).Conclusion:For children with pneumonia,inhaled NAC combined with glucocorticoid therapy could improve the clinical efficacy,improve the clinical indicators of children,and the overall safety is high.
