凉血通络颗粒辅助聚桂醇及大隐静脉高位结扎术对下肢静脉曲张患者术后康复及疼痛程度的影响

摘　　要：目的:观察凉血通络颗粒辅助聚桂醇对行大隐静脉高位结扎术下肢静脉曲张(湿热下注型)患者术后康复及疼痛的影响。方法:选取2020年5月-2021年12月期间本院收治的70例湿热下注型下肢静脉曲张患者作为研究对象。采用随机数字表法将患者分为对照组和试验组,每组各35例。对照组采用大隐静脉高位结扎术联合聚桂醇治疗;试验组在对照组基础上加用凉血通络颗粒。分析比较两组的临床疗效、术后恢复情况、症状改善情况[视觉模拟疼痛评分法(VAS)、静脉临床严重程度评分表(VCSS)]、血液流变学相关指标(血液黏度、纤维蛋白原、红细胞比容、红细胞沉降率)、炎症指标[白细胞介素-6(IL-6)、肿瘤坏死因子-α、基质金属蛋白酶-13(MMP-13)]以及复发情况。结果:试验组的治疗总有效率显著高于对照组(P<0.05)。随访显示,两组的复发率无显著差异(P>0.05)。试验组术后4w的VAS、VCSS评分均显著低于对照组(P<0.05);试验组住院及下床活动时间均显著短于对照组(P<0.05)。术后4w,试验组的血液黏度、纤维蛋白原、红细胞比容、红细胞沉降率均显著低于对照组(P<0.05)。术后4 w,两组的IL-6、TNF-α、MMP-13水平均显著降低,且试验组的血清IL-6、TNF-α、MMP-13水平均显著低于对照组(P<0.05)。结论:凉血通络颗粒辅助聚桂醇及大隐静脉高位结扎术治疗湿热下注型下肢静脉曲张,能提高疗效,减轻患者疼痛,缓解病情,抑制炎症反应,促进术后恢复。Objective:To observe the effect of Liangxuetongluo granules assisted with laurosinol on postoperative recovery and pain in patients with lower extremity varicose veins(damp-heat type)after high saphenous vein ligation.Methods:From May 2020 to December 2021,70 patients with lower extremity varicose veins treated in our hospital were selected as the study objects.Patients were divided into control group and experimental group with 35 cases in each group by random number table method.The control group was treated with high ligation of great saphenous vein combined with cinnamyl alcohol.The experimental group was supplemented with Liangxuetongluo granule on the basis of control group.Clinical efficacy,postoperative recovery,symptom improvement[visual analogue pain scale(VAS),intravenous clinical severity scale(VCSS)],hemorheological indicators(blood viscosity,fibrinogen,erythrocyte specific volume,erythrocyte precipitation rate),inflammatory indicators[interleukin-6(IL-6),tumor necrosis factor-α,matrix metalloproteinase-13(MMP-13)]and recurrence of the two groups were analyzed and compared.Results:The total effective rate of experimental group was significantly higher than that of control group(P<0.05).Follow-up showed that there was no significant difference in recurrence rate between the two groups(P>0.05).VAS and VCSS scores of experimental group were significantly lower than those of control group 4 weeks after surgery(P<0.05).The time of hospitalization and getting out of bed in experimental group was significantly shorter than that in control group(P<0.05).At 4 weeks after operation,the blood viscosity,fibrinogen,erythrocyte specific volume and erythrocyte sedimentation rate of experimental group were significantly lower than those of control group(P<0.05).At 4 weeks after surgery,the levels of IL-6,TNF-αand MMP-13 in both groups were significantly decreased,and the serum levels of IL-6,TNF-αand MMP-13 in test group were significantly lower than those in control group(P<0.05).Conclusion:Liangxuetongluo Granu
