常规心电图与TET在可疑稳定性冠心病中的诊断价值

摘　　要：目的:分析常规心电图与运动平板试验(TET)对可疑稳定性冠心病的诊断价值。方法:选取2021年2月至2024年7月在我院就诊的疑似稳定性冠心病患者60例为研究对象,均行常规心电图及TET检查,以冠状动脉造影(CAG)检查结果为“金标准”,分析常规心电图与TET对稳定性冠心病的诊断效能。结果:60例患者经CAG检查诊断,共41例(68.33%)患者确诊稳定性冠心病,经常规心电图检查诊断,共31例(63.33%)患者确诊;经TET检查诊断,共38例(75.00%)患者确诊。TET检查敏感度为82.93%、准确度为81.67、阴性预测值为68.18%均显著高于常规心电图的60.89%、56.67%、37.93%,差异具有统计学意义(P<0.05);TET检查的特异度为78.95%、阳性预测值为89.47%与常规心电图的68.42%、74.19%相比无显著性差异(P>0.05);常规心电图检查结果与CAG相比Kappa值为0.123,一致性差;TETKappa值为0.594,一致性较好。结论:对稳定性冠心病患者采用TET诊断准确率高于常规心电图,推荐对于入院就诊的可疑稳定性冠心病患者行TET检查。Objective:To analyze the diagnostic value of conventional electrocardiogram and treadmill exercise test(TET)in suspected stable coronary heart disease.Methods:Sixty patients with suspected stable coronary heart disease who visited the hospital from February 2021 to July 2024 were selected as the research subjects.All of them received conventional electrocardiography and TET.The results of coronary angiography(CAG)were used as the gold standard to analyze the diagnostic efficacy of conventional electrocardiogram and TET for stable coronary heart disease.Results:In this study,41 patients(68.33%),31 patients(63.33%)and 38 patients(75.00%)were diagnosed with stable coronary heart disease by CAG,conventional electrocardiogram and TET,respectively.The sensitivity,accuracy and negative predictive value of TET(82.93%,81.67%and 68.18%)were significantly higher than those of conventional electrocardiogram(60.89%,56.67%and 37.93%)(P<0.05).The specificity and positive predictive values of TET(78.95%and 89.47%)and conventional electrocardiogram(68.42%and 74.19%)were comparable(P>0.05).Compared with CAG,Kappa value of conventional electrocardiogram was 0.123,indicating poor consistency.Kappa value of TET was 0.594,indicating good consistency.Conclusion:The diagnostic accuracy of TET for stable coronary heart disease is higher than that of conventional electrocardiogram.Therefore,TET is recommended for patients with suspected stable coronary heart disease.
