摘 要:目的:探讨女性体质量指数(Body mass index,BMI)对夫精宫腔内人工授精(Intrauterine insemination,IUI)妊娠结局的影响。方法:选取2020年1月至2023年6月在本院生殖医学中心进行夫精IUI助孕治疗的不孕不育夫妇351例作为研究对象,对其647个周期的临床资料进行回顾性分析。根据临床妊娠与否,将患者分为妊娠组(n=115)和非妊娠组(n=532);根据女性BMI,将患者分为正常组、超重组和肥胖组,分析比较组间基本资料和妊娠结局。采用多因素Logistic回归分析BMI对临床妊娠率、早期流产率以及活产率的影响。结果:本研究共纳入符合标准的647个夫精IUI助孕周期,其中115个周期获得临床妊娠,临床妊娠率为17.77%。妊娠组患者的平均年龄更小,不孕年限更低,但妊娠组患者的BMI明显高于非妊娠组(P<0.05)。多因素Logistic回归分析结果显示,女方年龄(OR=0.946,95%CI:0.903~0.992,P=0.022)、不孕年限(OR=0.897,95%CI:0.809~0.994,P=0.038)、BMI高低(OR=1.733,95%CI:1.090~2.755,P=0.020)以及采用促排卵方案(OR=1.645,95%CI:1.052~2.574,P=0.029)是临床妊娠的影响因素(P<0.05)。此外,超重是早期流产的独立保护因素(OR=0.313,95%CI:0.103~0.949,P=0.040),而超重和肥胖则是活产的危险因素(OR=2.255,95%CI:1.349~3.768,P=0.002);(OR=2.077,95%CI:1.129~3.821,P=0.019)。结论:女性BMI对夫精IUI临床妊娠率存在显著影响,超重和肥胖患者的临床妊娠率和活产率较高,可依据BMI制定个性化的治疗方案。Objective:To explore the impact of female body mass index(BMI)on pregnancy outcomes of intrauterine insemination(IUI)with husband's sperm.Methods:A total of 351 infertile couples who underwent IUI treatment for assisted reproductive technology at the Reproductive Medicine Center of our hospital from January 2020 to June 2023 were selected as the research subjects,and their clinical data from 647 cycles were retrospectively analyzed.Patients were divided into a pregnancy group(n=115)and a non-pregnancy group(n=532)based on clinical pregnancy status;Based on the BMI of women,patients were divided into a normal group,overweight group,and obese group,and the basic data and pregnancy outcomes were analyzed and compared among the groups.Multivariate logistic regression analysis was used to examine the impact of BMI on clinical pregnancy rate,early abortion rate,and live birth rate.Results:A total of 647 cycles of assisted reproductive technology(ART)with intrauterine insemination(IUI)using the husband's sperm were included in this study,of which 115 cycles achieved clinical pregnancy,with a clinical pregnancy rate of 17.77%.The average age of patients in the pregnancy group was younger and the duration of infertility was shorter.Still,the BMI of patients in the pregnancy group was significantly higher than that of the non-pregnancy group(P<0.05).The results of the multi-factor logistic regression analysis showed that the age of the female partner(OR=0.946,95%CI:0.903~0.992,P=0.022),duration of infertility(OR=0.897,95%CI:0.809~0.994,P=0.038),BMI level(OR=1.733,95%CI:1.090~2.755,P=0.020),and use of ovulation induction protocol(OR=1.645,95%CI:1.052~2.574,P=0.029)were the influencing factors for clinical pregnancy(P<0.05).In addition,overweight was an independent protective factor for early miscarriage(OR=0.313,95%CI:0.103~0.949,P=0.040),while overweight and obesity were risk factors for live birth(OR=2.255,95%CI:1.349~3.768,P=0.002);(OR=2.077,95%CI:1.129~3.821,P=0.019).Conclusion:The BMI of women has a significant impact
