地奥司明片联合高锰酸钾片坐浴对混合痔术后创面愈合及肛门功能的影响

摘　　要：目的:观察地奥司明片联合高锰酸钾片坐浴对混合痔患者术后创面愈合及肛门功能的影响。方法:选取2022年7月至2024年8月于本院进行吻合器痔上黏膜环切手术(Procedure for prolapse and haemorrhoids,PPH)治疗的混合痔患者共62例,随机分成对照组与联合组,每组31例,对照组采用术后高锰酸钾片坐浴治疗,联合组采用术后地奥司明片联合高锰酸钾片坐浴治疗,观察比较两组患者疗效、术后创面愈合情况以及肛门功能。结果:治疗2 w后,联合组治疗总有效率明显高于对照组(P<0.05)。相较于对照组,联合组的肉芽组织与创面相平时间、创面愈合时间更短(P<0.05)。治疗后,两组患者肛肠动力学指标水平(肛管静息压、肛管舒张压与肛管最大收缩压)均明显升高,Wexner评分均显著下降,相比于对照组,联合组肛肠动力学指标水平更高,Wexner评分更低(P<0.05)。两组并发症总发生率无显著差异(P>0.05)。结论:混合痔患者行PPH术后使用地奥司明片联合高锰酸钾片坐浴治疗疗效较好,能有效促进创面愈合,患者肛门功能恢复更好。Objective:To observe the effect of diosmin tablets combined with potassium permanganate sitz bath on wound healing and anal function in patients after surgery for mixed hemorrhoids.Methods:A total of 62 patients with mixed hemorrhoids who underwent procedure for prolapse and hemorrhoids(PPH)at the hospital from July 2022 to August 2024 were selected,and randomized into a control group and a combination group,with 31 cases in each group.The control group was treated with potassium permanganate sitz bath after surgery,while the combination group was treated with diosmin tablets combined with potassium permanganate sitz bath after surgery.Postoperative wound healing and anal function were observed and compared between groups.Results:After 2 weeks of treatment,the total effective rate of the combination group was significantly higher than that of the control group(P<0.05).The time for granulation tissue to be level with the wound and wound healing time in the combination group were significantly shorter(P<0.05).After treatment,the levels of anorectal motility indicators(anal resting pressure,anal diastolic pressure,and maximum anal systolic pressure)in both groups of patients significantly increased,and the Wexner scores significantly decreased.Compared with the control group,the combined group had higher levels of anorectal motility indicators and lower Wexner scores(P<0.05).The total incidence of complications showed no significant difference between groups.Conclusion:The combined treatment with diosmin tablets and potassium permanganate sitz bath can achieve good therapeutic effects on patients with mixed hemorrhoids after PPH.It can effectively promote wound healing and improve anal function.
