超声引导下星状神经节阻滞联合静吸麻醉在妇科肿瘤根治性手术患者中的应用

摘　　要：目的:探讨超声引导下星状神经节阻滞(SGB)联合静吸麻醉在妇科肿瘤根治性手术患者中的应用效果。方法:回顾性收集2021年12月-2023年12月期间本院收治的60例行妇科肿瘤根治性手术治疗患者的临床资料。根据不同麻醉方式将患者分为对照组(静吸复合麻醉,45例)和观察组(超声引导下SGB联合静脉吸入复合麻醉,46例)。分析比较两组入室后(T_(0))、气管插管(T_(1))、手术开始(T_(2))、手术结束时(T_(3))心率和平均动脉压水平;苏醒时间、恢复自主呼吸时间、术后6 h、12 h、24 h疼痛程度[视觉模拟评分法(VAS)评分]、镇静程度(Ramsay评分)以及术后不良反应发生情况。结果:两组的心率、平均动脉压组间、时点、组间*时点交互比较均有显著差异(P<0.05)。两组术后苏醒时间、恢复自主呼吸时间均无显著差异(P>0.05)。两组术后VAS评分、Ramsay评分组间、时点、组间*时点交互比较均有显著差异(P<0.05)。两组麻醉苏醒期间不良反应发生率无显著差异(P>0.05)。结论:超声引导下SGB联合静吸麻醉能改善妇科肿瘤根治性手术患者生理应激反应,提高镇痛、镇静效果,且不会增加麻醉不良反应。Objective:To investigate the effect of ultrasound-guided stellate ganglion block(SGB)combined with inhalation anesthesia in patients undergoing radical surgery for gynecological tumors.Methods:The clinical data of 60 patients who underwent radical surgical treatment for gynecological tumors in our hospital from December 2021 to December 2023 were retrospectively collected.According to different anesthesia methods,patients were divided into control group(intravenous inhalation combined anesthesia,45 cases)and observation group(ultrasound-guided SGB combined with intravenous inhalation combined anesthesia,46 cases).The heart rate and mean arterial pressure after entry(T0),tracheal intubation(T1),at the beginning of surgery(T2)and at the end of surgery(T3),time to recovery,time to spontaneous breathing,degree of pain(VAS)at 6 hours,12 hours,and 24 hours after surgery,degree of sedation(Ramsay score),and incidence of adverse reactions of the two groups were analyzed and compared.Results:There were significant differences in heart rate,mean arterial pressure between groups,time point and*time point between groups(P<0.05).There were no significant differences between the two groups(P>0.05)in recovery time and spontaneous breathing time.There were significant differences in VAS score,Ramsay score between groups,time point and*time point interaction between groups between the two groups(P<0.05).There was no significant difference in the incidence of adverse reactions during anesthesia recovery between the two groups(P>0.05).Conclusion:Ultrasound-guided SGB combined with intravenous anesthesia can improve the physiological stress response of patients undergoing radical surgery for gynecological tumors,improve analgesia and sedation effects,and do not increase the side effects of anesthesia.
