中医适宜技术综合干预联合司美格鲁肽治疗2型糖尿病合并超重/肥胖的临床疗效研究

摘　　要：目的分析中医适宜技术综合干预联合司美格鲁肽治疗2型糖尿病合并超重/肥胖的临床疗效。方法选取2022年10月—2023年10月青海红十字医院中医科(中西医结合糖尿病科)收治的90例2型糖尿病合并超重/肥胖的患者为研究对象,按治疗方法的不同分为对照组和观察组,各45例。对照组在生活方式干预的基础上予司美格鲁肽注射液治疗,观察组在对照组基础上采用耳穴压丸、穴位按摩及八段锦中医适宜技术综合干预治疗。对比两组的肥胖指标[体重指数(body mass index,BMI)及腰围(waist circumference,WC)]、血糖指标[糖化血红蛋白(glycated hemoglobin A1c,HbA1c)、空腹血糖(fasting plasma glucose,FPG)、餐后2 h血糖(2-hour postprandial plasma glucose,2 hPG)]、血脂指标[总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)及低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)]、胰岛素抵抗指数(homeostasis mode assessment of insulin resistance,HOMA-IR)以及不良反应发生率。结果治疗后,两组BMI、WC、FPG、2 hPG、HbA1c、TC、TG、LDL-C及HOMA-IR水平均低于治疗前,观察组BMI、WC、FPG、2 hPG、HbA1c、TG水平均低于对照组,差异均有统计学意义(P均<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论在2型糖尿病合并超重/肥胖的患者中使用中医适宜技术综合干预联合司美格鲁肽注射液治疗,可有效减轻患者的体重,并能改善糖脂代谢水平及胰岛素抵抗。Objective To analyze the clinical effect of comprehensive intervention of appropriate technology of tradi-tional Chinese medicine combined with semeglutide in the treatment of type 2 diabetes mellitus complicated with over-weight/obesity.Methods A total of 90 patients with type 2 diabetes mellitus complicated with overweight/obesity who were admitted to the Department of Traditional Chinese Medicine(Diabetes Department of Integrated Traditional Chi-nese and Western Medicine)of Qinghai Red Cross Hospital from October 2022 to October 2023 were selected as the research objects.According to different treatment methods,they were divided into the control group and the observa-tion group,with 45 cases in each group.The control group was treated with semaglutide injection on the basis of life-style intervention,and the observation group was treated with auricular acupoint pressure pills,acupoint massage and Baduanjin traditional Chinese medicine appropriate technology comprehensive intervention on the basis of the control group.The obesity indexes[body mass index(BMI)and waist circumference(WC)],blood glucose indexes[glycated hemoglobin A1c(HbA1c),fasting plasma glucose(FPG),2-hour postprandial plasma glucose(2 hPG)],blood lipid pa-rameters[total cholesterol(TC),triglyceride(TG)and low density lipoprotein cholesterol(LDL-C)],homeostasis model assessment of insulin resistance(HOMA-IR)and incidence of adverse reactions were compared between the two groups.Results After treatment,levels of BMI,WC,FPG,2 hPG,HbA1c,TC,TG,LDL-C and HOMA-IR of the two groups were lower than those before treatment,and levels of BMI,WC,FPG,2 hPG,HbA1c and TG of the observation group were lower than those of the control group,and the differences were statistically significant(all P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Comprehensive intervention with appropriate traditional Chinese medicine technology combined with semaglutide injection can effectively
