摘　　要：目的探讨胫骨骨膜牵张术在糖尿病足患者治疗中的应用效果。方法选取2022年1月—2024年5月芜湖市第一人民医院收治的80例糖尿病足患者,根据治疗方式的不同分为对照组(40例,常规清创负压引流治疗)和观察组(40例,胫骨骨膜牵张术治疗),比较两组足部相关指标、炎症指标、创面情况、临床疗效及并发症发生率。结果治疗后,观察组足温、趾端血氧、踝肱指数、足背动脉管腔内径、足背平均血流速度均高于对照组,白细胞计数、降钙素原、白细胞介素-6、超敏C反应蛋白水平均低于对照组,创面面积、创面深度均小于对照组,差异均有统计学意义(P均<0.05)。观察组治疗总有效率高于对照组,并发症发生率低于对照组,差异均有统计学意义(P均<0.05)。结论胫骨骨膜牵张术治疗不仅能够有效改善糖尿病足患者的症状,减轻炎症反应,还能够促进创面愈合,提升治疗效果,减少并发症的发生。Objective To explore the application effect of tibial periosteal distraction in the treatment of diabetic foot patients.Methods A total of 80 patients with diabetic foot admitted to the First People's Hospital of Wuhu from Janu-ary 2022 to May 2024 were selected.According to different treatment methods,they were divided into control group(40 cases,conventional debridement and negative pressure drainage treatment)and observation group(40 cases,tibial periosteum distraction treatment).The foot related indexes,inflammation indexes,wound conditions,clinical efficacy and complications of the two groups were compared.Results After treatment,the foot temperature,toe end blood oxy-gen,ankle Brachial Index,dorsalis pedis artery lumen diameter and dorsalis pedis average blood flow velocity in the observation group were higher than those in the control group,the levels of white blood cell count,procalcitonin,inter-leukin-6 and hypersensitivity C-reactive protein were lower than those in the control group,and the wound area and wound depth were smaller than those in the control group,the differences were statistically significant(all P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group,and the inci-dence of complications was lower than that of the control group,the differences were statistically significant(both P<0.05).Conclusion The treatment of tibial periosteal distraction can not only effectively improve the symptoms of dia-betic foot patients,reduce the inflammatory response,but also promote wound healing,improve the therapeutic effect and reduce the occurrence of complications.
