糖尿病患者空腹胰岛素、空腹C肽、空腹血糖及糖化血红蛋白联合检测的诊断价值

摘　　要：目的分析空腹胰岛素、空腹C肽、空腹血糖及糖化血红蛋白联合检测对糖尿病患者的诊断价值。方法选取2022年8月—2024年5月南京市高淳人民医院的98例糖尿病患者为疾病组,选取同期体检的67例健康者为健康组。对比两组空腹胰岛素、空腹C肽、空腹血糖及糖化血红蛋白水平变化,并利用受试者操作特征(receiver operating characteristic,ROC)曲线评估以上指标在糖尿病中的诊断价值。结果疾病组空腹胰岛素、空腹C肽、空腹血糖及糖化血红蛋白水平均显著高于健康组,差异均有统计学意义(P均<0.05)。ROC曲线分析显示,空腹胰岛素、空腹C肽、空腹血糖及糖化血红蛋白联合检测的灵敏度为96.9%,特异度为97.0%,曲线下面积为0.982,联合检测的诊断价值较高。结论空腹胰岛素、空腹C肽、空腹血糖及糖化血红蛋白联合检测对糖尿病的诊断具有重要价值,能够有效提高诊断的灵敏度和特异度。Objective To analyze the diagnostic value of fasting insulin,fasting C-peptide,fasting plasma glucose and glycated hemoglobin A1c in patients with diabetes meuitus.Methods A total of 98 patients with diabetes mellitus in Nanjing Gaochun People's Hospital from August 2022 to May 2024 were selected as the disease group,and 67 healthy patients who underwent physical examination during the same period were selected as the healthy group.The levels of fasting insulin,fasting C-peptide,fasting plasma glucose and glycated hemoglobin A1c were compared be-tween the two groups,and the diagnostic value of the above indexes in diabetes mellitus was evaluated by receiver op-erating characteristic(ROC)curve.Results The levels of fasting insulin,fasting C-peptide,fasting plasma glucose and glycated hemoglobin A1c in disease group were significantly higher than those in healthy group,and the differences were statistically significant(all P<0.05).ROC curve analysis showed that the sensitivity,specificity and area under the curve of the combined detection of fasting insulin,fasting C-peptide,fasting plasma glucose and glycated hemoglo-bin A1c were 96.9%,97.0%and 0.982,indicating high diagnostic value of the combined detection.Conclusion The combined detection of fasting insulin,fasting C-peptide,fasting plasma glucose and glycated hemoglobin A1c is of great value in the diagnosis of diabetes mellitus,and can effectively improve the sensitivity and specificity of diagnosis.
