摘  要:目的分析布地奈德雾化吸入与静脉注射甲泼尼龙对慢性阻塞性肺疾病合并2型糖尿病患者的治疗效果。方法回顾性选取2023年2月—2024年6月泉州市光前医院收治的106例慢性阻塞性肺疾病合并2型糖尿病患者的临床资料,以不同的治疗方式分为两组,各53例。两组均行糖尿病、慢性阻塞性肺疾病常规治疗,在此基础上,对照组采用静脉注射甲泼尼龙治疗,观察组采用布地奈德雾化吸入治疗。对比两组肺功能、糖代谢水平、血糖波动情况。结果干预后,两组最大呼气流速、第1秒用力呼气容积/用力肺活量均高于干预前,差异均有统计学意义(P均<0.05);两组组间比较,差异均无统计学意义(P均>0.05)。干预后,观察组空腹血糖、餐后2 h血糖、空腹胰岛素水平均低于对照组,差异均有统计学意义(P均<0.05)。干预后,观察组血糖波动情况优于对照组,差异有统计学意义(P<0.05)。结论甲泼尼龙、布地奈德两种药物均可改善慢性阻塞性肺疾病合并2型糖尿病患者的肺功能,但从糖代谢影响角度来看,布地奈德对患者血糖波动影响较为轻微。Objective To analyze the therapeutic effect of budesonide aerosol inhalation and intravenous injection of methylprednisolone on patients with chronic obstructive pulmonary disease complicated with type 2 diabetes mellitus.Methods The clinical data of 106 patients with chronic obstructive pulmonary disease complicated with type 2 diabe-tes mellitus admitted to Quanzhou Guangqian Hospital from February 2023 to June 2024 were retrospectively se-lected.The patients were divided into two groups according to different treatment methods,with 53 cases in each group.Both groups were treated with routine treatment of diabetes and chronic obstructive pulmonary disease.On this basis,the control group was treated with intravenous methylprednisolone,and the observation group was treated with budesonide aerosol inhalation.The lung function,glucose metabolism levels and blood glucose fluctuation status were compared between the two groups.Results After intervention,the maximum expiratory flow rate and forced expiratory volume in the first second/forced vital capacity of the two groups were higher than those before intervention,and the differences were statistically significant(both P<0.05);there were no significant differences between the two groups(both P>0.05).After intervention,the levels of fasting plasma glucose,2-hour postprandial plasma glucose and fasting insulin in the observation group were lower than those in the control group,and the differences were statistically sig-nificant(all P<0.05).After intervention,the blood glucose fluctuation status in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Both methylpredniso-lone and budesonide can improve the lung function of patients with chronic obstructive pulmonary disease compli-cated with type 2 diabetes mellitus.However,from the perspective of glucose metabolism,budesonide has a slight ef-fect on blood glucose fluctuation.
