基于吴澄“理脾阴”思想浅析保金汤加减治疗晚期肺癌

摘　　要：脾胃居于中焦,主生化气血,同为气机升降的枢纽,脾阴亏虚,气血的生成及运行失衡,脏腑失于濡润,气血壅滞,痰、瘀、毒等形成是晚期肺癌的病机关键。吴澄首次提出“脾虚有阴阳之分”,倡导“理脾以健胃,补阴以扶阳”。基于吴澄“理脾阴”思想认为临床中应在补脾胃阳气的同时,兼顾养脾阴,使用芳香甘淡之品,培土生金,燥润相宜,以吴澄自制保金汤加减,辅以血肉有情之品,从而使脏腑气血功能协调,则痰、瘀、毒等渐消缓散,为晚期肺癌的中医治疗提供一种新的思路与方法。The spleen and stomach are located in the middle energizer and are responsible for Qi and blood.They are both the hub for the rise and fall of Qi and blood.Spleen Yin deficiency,imbalance in the generation and operation of Qi and blood,loss of nourishment in the organs,stagnation of Qi and blood,and the formation of phlegm,blood stasis and toxins are the key pathogenesis of advanced lung cancer.WU Cheng proposed for the first time that“spleen deficiency can be divided into Yin deficiency and Yang deficiency”,advocating“regulating spleen to strengthen stomach,supplementing Yin to support Yang”.Based on WU Cheng′s idea of“regulating spleen Yin”,it is believe that in clinical practice,the treatment should not only supplement Yang of spleen and stomach,but also nourish spleen Yin.And aromatic,sweet and light products,reinforcing earth to generate metal,and drying and moistening methods should be used appropriately,supplemented by products with blood and flesh emotions,so as to coordinate Qi and blood functions of Zang-Fu organs,and gradually dissipate phlegm,blood stasis and toxins,providing a new approach and method for the traditional Chinese medicine treatment of advanced lung cancer.
