从“脾胃与小肠相关”探析糖尿病胃轻瘫

摘　　要：糖尿病胃轻瘫是继发于糖尿病常见的慢性消化道并发症之一,常表现为恶心呕吐、腹胀、食欲不振等,因其病程漫长、发病隐匿,易累及胃肠部的病变,严重影响患者生活质量。糖尿病胃轻瘫病因病机虽复杂,但根据临床表现,中医学认为其病位在脾胃,日久累及小肠病变,因脾胃与小肠在生理功能及病理方面相似,一脏一腑同主运化之功,共同完成对饮食的消化吸收以及对精微物质的转运,故该文从“脾胃与小肠相关”探析糖尿病胃轻瘫,并从理论渊源、脏腑相连、生理及病理等方面深层次剖析糖尿病胃轻瘫的发病机理,为防治糖尿病胃轻瘫提供新思路和方法。Diabetic gastroparesis is one of the common chronic gastrointestinal complications secondary to diabetes,often manifested as nausea and vomiting,abdominal distension,loss of appetite,etc.Because of its long course of disease,insidious onset and being easy to involve gastrointestinal lesions,it seriously affects the quality of life of patients.Although the etiology and pathogenesis of diabetic gastroparesis are complex,according to the clinical manifestations,traditional Chinese medicine believes that the disease is located in spleen and stomach,which affects the small intestine over time.Because spleen,stomach and small intestine are similar in physiological function and pathology,and they work together to complete the digestion and absorption of food and drink and the transportation of subtle substances,so this paper explored the pathogenesis of diabetic gastroparesis from the aspects of“spleen and stomach and small intestine being related”,and analyzed the pathogenesis of diabetic gastroparesis from the aspects of theoretical origin,viscera connection,physiology and pathology.It provided new ideas and methods for the prevention and treatment of diabetic gastroparesis.
