解郁散结方治疗女性气滞血瘀型乳腺良性结节的临床研究

摘　　要：目的评价解郁散结方治疗女性气滞血瘀型乳腺良性结节的有效性及安全性。方法采用自身前后对照设计,治疗周期为6个月。比较观察周期内解郁散结方对气滞血瘀型乳腺结节的治疗效果和安全性。结果共纳入84例患者,与治疗前相比,治疗3个月、治疗6个月乳腺疼痛,视觉模拟评分量表(visual analogue scale,VAS)评分均显著下降(P<0.05);与治疗前相比,治疗3个月、治疗6个月中医症状明显改善(P<0.05);乳腺结节最大径线有下降趋势。治疗3个月、治疗6个月焦虑自评量表(self-rating anxiety scale,SAS),抑郁自评量表(self-rating depression scale,SDS)评分均较治疗前下降,差异有统计学意义(P<0.05)。E_(2)/P水平治疗3个月较治疗前有下降趋势。治疗过程中无明显不良反应发生。结论解郁散结方治疗乳腺良性结节可有效缓解乳房疼痛,延缓结节进展,明显调节气滞血瘀型患者中医证候和心理状态,减轻患者焦虑抑郁程度,且不影响E 2/P表达水平,具有较显著的临床疗效和安全性。Objective To evaluate the efficacy and safety of Jieyu Sanjie Recipe(解郁散结方)in the treatment of female benign breast nodules with Qi stagnation and blood stasis type.Methods Self-control comparison was adopted with a treatment period of 6 months.In treating mammary nodules of Qi stagnation and blood stasis type,the clinical efficacy and safety of Jieyu Sanjie Recipe were observed and compared.Results After 3and 6 monthstreatment,the visual analogue scale(VAS)scores of 84 patients were significantly decreased(P<0.05),respectively.After 3and 6 monthstreatment,the symptoms of TCM were obviously improved(P<0.05),respectively.The maximum diameter of breast nodules tended to lessen.After 3and 6 monthstreatment,the scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)were significantly decreased(P<0.05),respectively.The levelof estradiol(E 2)/progesterone(P)was also decreased after 3 monthstreatment.No obvious adverse reaction occurred during the treatment.Conclusion Jieyu Sanjie Recipe can effectively relieve breast pain,delay the development of breast nodules,obviously regulate the TCM syndrome and psychological state of patients with Qi stagnation and blood stasis,and relieve their anxiety and depression.Besides,the level of E_(2)/P was not affected,and the clinical efficacy and safety were significant.
