丹栀逍遥散联合五行音乐疗法治疗轻度卒中后抑郁临床研究

摘　　要：目的观察丹栀逍遥散联合五行音乐疗法治疗轻度卒中后抑郁(post-stroke depression,PSD)的临床疗效。方法选取80例轻度PSD患者随机分为试验组(40例)和对照组(40例)。对照组予口服盐酸舍曲林片治疗;试验组在对照组基础上予丹栀逍遥散联合五行音乐疗法治疗。比较两组临床疗效、汉密尔顿抑郁评分量表(Hamilton depression rating scale,HDRS)评分、Barthel指数(Barthel index,BI)评分、美国国立卫生研究院卒中量表(National Institutes of Health stroke scale,NIHSS)评分、血清脑源性神经营养因子(brain derived neurotrophic factor,BDNF)水平、血清胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)水平以及不良反应发生率。结果治疗后,两组HDRS评分、NIHSS评分、血清IGF-1水平均低于治疗前(P<0.01),BI评分、血清BDNF水平均高于治疗前(P<0.01);试验组HDRS评分、NIHSS评分、血清IGF-1水平均低于对照组(P<0.01),BI评分、血清BDNF水平均高于对照组(P<0.05或P<0.01)。试验组临床有效率高于对照组[95.00%(38/40)vs 77.50%(31/40)](P<0.05)。试验组不良反应发生率低于对照组[10.00%(4/40)vs 30.00%(12/40)](P<0.05)。结论丹栀逍遥散联合五行音乐疗法能够有效改善轻度PSD患者的抑郁程度、神经功能和日常生活活动能力,提高神经营养因子合成释放,临床疗效显著且安全性评价良好,值得临床进一步推广应用。Objective To observe the clinical efficacy of Danzhi Xiaoyao Powder(丹栀逍遥散,DZXYD)combined with five-element music therapy in the treatment of mild post-stroke depression(PSD)patients.Methods Eighty patients with mild PSD were randomly divided into experimental group(40 cases)and control group(40 cases).The control group was treated with Sertraline Hydrochloride Tablet orally.The experimental group was treated with DZXYD combined with five-element music therapy on the basis of the control group.The clinical efficacy,Hamilton depression rating scale(HDRS)score,Barthel index(BI)score,National Institutes of Health stroke scale(NIHSS)score,serum brain derived neurotrophic factor(BDNF)level,serum insulin-like growth factor-1(IGF-1)level and the incidence of adverse reactions were compared between the two groups.Results After treatment,HDRS score,NIHSS score and serum IGF-1 level were lower while BI score and serum BDNF level were higher than those before treatment in both groups(P<0.01).HDRS score,NIHSS score and serum IGF-1 level in the experimental group were lower while BI score and serum BDNF level were higher than those in the control group(P<0.05,P<0.01).The clinical effective rate of the experimental group was higher than that of the control group[95.00%(38/40)vs 77.50%(31/40)](P<0.05).The incidence of adverse reactions in the experimental group was lower than that in the control group[10.00%(4/40)vs 30.00%(12/40)](P<0.05).Conclusion DZXYD combined with five-element music therapy can effectively improve depression degree,neurological function,activities of daily living ability,synthesis and release of neurotrophin in patients with mild PSD,which has significant clinical efficacy and good safety evaluation,and is worthy of further clinical application.
