化瘀涤痰汤改善中风偏瘫患者肢体运动功能的效果  

Effect of Huayu Ditan Decoction(化瘀涤痰汤)on Limb Motor Function of Patients with Stroke Hemiplegia

摘　　要：目的分析化瘀涤痰汤改善中风偏瘫患者肢体运动功能的效果。方法选取99例阜阳市妇女儿童医院收治的中风偏瘫患者,分为对照组(49例)和研究组(50例),分组方法为随机数字表法,选例时间设置在2018年1月-2022年12月。对照组采用西医药物及运动疗法治疗,研究组在对照组的基础上采用化瘀涤痰汤治疗,两组患者连续治疗1个月。比较两组治疗后临床疗效及治疗期间不良反应发生情况,治疗前后中医证候积分、神经功能、肢体运动功能、神经营养及炎症因子指标。结果治疗后,研究组总有效率高于对照组;治疗后与治疗前比较,两组中医证候各项积分、美国国家卫生研究院卒中量表(national institute of health stroke scale,NIHSS)评分、血清CXC趋化因子16(CXC chemokineligand 16,CXCL-16)、基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)水平降低,两组间治疗后相比,研究组更低;治疗后与治疗前比较,两组肢体运动功能Fugl-Meyer(fugl-meyer,FMA)评分、Brunnstrom分期量表评分、血清脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)、神经生长因子(nerve growth factor,NGF)水平升高,研究组更高(两组间治疗后相比),差异有统计学意义(P<0.05)。两组间不良反应总发生率(治疗期间)相比,差异无统计学意义(P>0.05)。结论化瘀涤痰汤治疗中风偏瘫有助于缓解患者的临床症状,调节神经营养及炎症因子指标水平表达,改善神经功能及肢体运动功能,提高临床疗效,且安全性高。Objective To analyze the effect of Huayu Ditan Decoction(化瘀涤痰汤)on improving limb motor function in patients with stroke hemiplegia.Methods Ninety-nine patients with stroke hemiplegia admitted to Fuyang Women and Children′s Hospital were divided into the control group(49 cases)and the research group(50 cases).The grouping method was the random number table method,and the selection time was set from January 2018 to December 2022.The control group was treated with western medicine and exercise therapy,while the research group was treated with Huayu Ditan Decoction on the basis of the control group.The two groups were treated continuously for 1 month.The clinical efficacy after treatmentand the occurrence of adverse reactions during treatment,TCM syndrome scores,neurological function,limb motor function,neurotrophic and inflammatory factors before and after treatment were compared between the two groups.Results After treatment,the total effective rate of the research group was higher than that of the control group.After treatment,compared with those before treatment,the scores of TCM syndromes,national institute of health stroke scale(NIHSS),the levels of serum CXC chemokineligand 16(CXCL-16)and matrix metalloproteinase-9(MMP-9)of the two groups decreased,and thescores and levels of the research group werelower than those of the control group.After treatment,compared with those before treatment,the fugl-meyer(FMA)score of limb motor function,Brunnstrom staging scale score,serum levels of brain-derived neurotrophic factor(BDNF)and nerve growth factor(NGF)of the two groups increased,and thescores and levels of the research group were higher than those of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups during treatment(P>0.05).Conclusion Huayu Ditan Decoction is helpful to relieve the clinical symptoms of patients with stroke hemiplegia,regulate the expressions of neurotrophic a
