益肾除颤汤改善帕金森病患者认知功能的研究

摘　　要：目的分析益肾除颤汤改善帕金森病患者认知功能的效果。方法选取89例苏州市中西医结合医院收治的帕金森病患者,分为对照组(44例)和研究组(45例),分组方法为随机数字表法,选例时间设置在2019年1月-2022年9月。两组均予以常规训练,同时对照组采用多巴丝肼片治疗,研究组在对照组的基础上采用益肾除颤汤治疗,两组患者连续治疗2个月。比较两组治疗后临床疗效及治疗期间不良反应发生情况,治疗前后中医证候评分、运动功能及认知功能、神经递质、神经营养因子指标。结果治疗后,研究组总有效率高于对照组;治疗后与治疗前比较,两组中医证候各项积分、帕金森病统一评分量表第三部分(unified Parkinson′s disease rating scaleⅢ,UPDRS-Ⅲ)评分、简易精神状态量表(mini mental state scale,MMSE)评分降低,两组间治疗后相比,研究组更低;治疗后两组蒙特利认知评估量表(monterey cognitive assessment scale,MoCA)评分、5-羟色胺(5-hydroxytryptamine,5-HT)、多巴胺(dopamine,DA)、乙酰胆碱(acetylcholine,ACh)、血清脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)、胶质细胞源性神经营养因子(glial cell-derived neurotrophic factor,GDNF)、神经生长因子(nerve growth factor,NGF)水平升高,两组间治疗后相比,研究组水平更高,差异均有统计学意义(P<0.05)。两组间不良反应总发生率(治疗期间)相比,差异无统计学意义(P>0.05)。结论帕金森病患者应用益肾除颤汤治疗,有助于提高临床疗效,缓解其临床症状,改善运动及认知功能,调节神经递质指标(5-HT、DA、ACh)、神经营养因子指标(血清BDNF、GDNF、NGF水平)表达水平,且安全性高。Objective To analyze the effect of Yishen Chuchan Decoction(益肾除颤汤)on improving cognitive function in patients with Parkinson′s disease.Methods Eighty-nine patients with Parkinson′s disease in Suzhou Hospital of Integrated Traditional Chinese and Western Medicine were selected and divided into the control group(44 cases)and the research group(45 cases).The grouping method was random number table method,and the selection time was set from January 2019 to September 2022.Both groups were given routine training,while the control group was treated with dobasizine tablets,and the research group was treated with Yishen Chuchan Decoction on the basis of the control group.The two groupswere treated continuously for 2 months.The clinical efficacy after treatment and the occurrence of adverse reactions during treatment,TCM syndrome scores,motor function and cognitive function,neurotransmitters and neurotrophic factors before and after treatment were compared between the two groups.Results After treatment,the total effective rate of the research group was higher than that of the control group.After treatment,compared with those before treatment,the scores of TCM syndromes,unified Parkinson′s disease rating scaleⅢ(UPDRS-Ⅲ)score and mini mental state scale(MMSE)score of the two groups decreased,and the scores of the research group were lower than those of the control group.After treatment,the levels of monterey cognitive assessment scale(MoCA)score,5-hydroxytryptamine(5-HT),dopamine(DA),acetylcholine(ACh),serum brain-derived neurotrophic factor(BDNF),glial cell-derived neurotrophic factor(GDNF)and nerve growth factor(NGF)of the two groups increased,and the levels of the research group were higher than those of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups during treatment(P>0.05).Conclusion Yishen Chuchan Decoction is helpful to relieve the clinical symptoms of Parkinson′s d
