自拟黄竭散促进混合痔术后创面愈合的临床观察

摘　　要：目的探讨自拟黄竭散应用于混合痔术后对患者创面愈合的影响。方法以随机数字表法将2023年2月-2023年10月于该院接受手术治疗的混合痔患者70例分为两组,分别命名为A组、B组,各35例。A组使用凡士林纱条外敷换药,B组给予自拟黄竭散外用换药。两组均连续干预15 d。比较两组干预15 d后两组肉芽组织生长情况、创面愈合率;干预前及干预1周后两组血清血管内皮生长因子(vascular endothelial growth factor,VEGF)、表皮生长因子(epidermal growth factor,EGF)、血小板源性生长因子(platelet-derived growth factor,PDGF)表达水平;干预前及干预15 d后两组创面面积、创面恢复指标评分;创面愈合速度;干预期间不良反应发生情况。结果干预15 d后,比较A、B两组肉芽组织生长情况,后者优良率高于前者;干预1周后,两组血清EGF、VEGF、PDGF水平均较干预前有显著提升,且B组表达水平高于A组;干预15 d后,两组创面面积较干预前均有缩小,B组小于A组;干预15 d后B组创面愈合率高于A组;B组创面愈合速度快于A组;干预15 d后,两组创面疼痛、水肿、渗液评分均较干预前显著下降,且B组低于A组;均P<0.05。干预期间,B组不良反应总发生率略低于A组,P>0.05。结论自拟黄竭散用于混合痔术后可提高各类生长因子表达,促进患者肉芽组织生长,加快创面愈合,安全性高。Objective To explore the effect of Self-Made Huangjie Powder(自拟黄竭散)on wound healing after mixed hemorrhoid operation.Methods Seventy patients with mixed hemorrhoids who underwent operation treatment in the hospital from February 2023 to October 2023 were divided into two groups by the random number table method,named the group A and the group B respectively,with 35 cases in each group.The group A was treated with vaseline gauze and the group B was treated with Self-made Huangjie Powder.Both groups were continuously intervened for 15 days.The granulation tissue growth and wound healing rate after 15 days of intervention,the expression levels of vascular endothelial growth factor(VEGF),epidermal growth factor(EGF)and platelet-derived growth factor(PDGF)in serum of the two groups before and after 1 week of intervention,wound area and wound recovery index scores of the two groups before and after 15 days of intervention,wound healing speed and the incidence of adverse reactions during intervention of the two groups were compared.Results After 15 days of intervention,the growth of granulation tissue of the group A and the group B was compared,and the excellent and good rate of the latter was higher than that of the former.After 1 week of intervention,the levels of serum EGF,VEGF and PDGF of the two groups were significantly higher than those before intervention,and the expression level of the group B was higher than that of the group A.After 15 days of intervention,the wound area of the both groups was smaller than that before intervention,and the wound area of the group B was smaller than that of the group A.After 15 days of intervention,the wound healing rate of the group B was higher than that of the group A.The wound healing speed of the group B was faster than that of the group A.After 15 days of intervention,the scores of wound pain,edema and exudation of the both groups were significantly lower than those before intervention,and the scores of the group B were lower than those of the group A(all P<0.05
