基于数据挖掘探析中医治疗癌性发热的用药规律

摘　　要：目的:研究中医治疗癌性发热的用药规律,为临床提供参考。方法:收集辽宁中医药大学附属医院癌性发热患者临床处方,对收集到的复方进行频数、关联规则、聚类和因子分析等。结果:共收集处方60首,涉及中药137味,累计频数为1383。其中,使用频数≥14的中药有33味,以炙甘草(4.12%,57/1383)最为常见。药性以寒(27.11%,375/1383)、温(23.43%,324/1383)为主,药味以甘(33.16%,757/2283)、苦(25.27%,577/2283)为主,药物功效以补虚和清热为主,归经以归肺经(22.42%,868/3871)最多、脾经(17.72%,686/3871)次之。通过高频药物聚类分析得到5类药物组合,关联规则分析得到13对高频药对组合及13对置信度较高的关联组合,因子分析得到9个公因子。结论:中医治疗癌性发热以补益药、清热药使用频率最高,主要采用益气养阴、清热泻火配合软坚散结法治疗,处方以竹叶石膏汤、四物汤为核心。Objective:To study the medication rules of traditional Chinese medicine in the treatment of neoplastic fever,so as to provide reference for clinical practice.Methods:The clinical prescriptions of neoplastic fever patients in the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were collected,and the collected compounds,frequency,association rules,clustering and factor analyses were performed.Results:A total of 60 prescriptions were collected,including 137 traditional Chinese medicines,with a cumulative frequency of 1383 times.Among them,thirty-three traditional Chinese medicines with frequency≥14 were used,among which roasted Gancao(Radix Glycyrrhizae)(4.12%,57/1383)was the most common.The medicinal properties were mainly cold(27.11%,375/1383),warm(23.43%,324/1383),and the medicinal flavor was mainly sweet(33.16%,757/2283)and bitter(25.27%,577/2283).The most important drugs were used for tonifying and heat clearing.The lung meridian(22.42%,868/3871)was the most important meridian,followed by the spleen meridian(17.72%,686/3871).Five drug combinations were obtained by high-frequency drug clustering analysis,a total of 13 pairs of high-frequency drug combinations and 13 pairs of association combinations with high confidence were obtained by association rule analysis,and 9 common factors were obtained by factor analysis.Conclusion:In the treatment of neoplastic fever,tonic drugs and heat-clearing drugs have the highest frequency,mainly using the methods of nourishing Qi(气)and Yin(阴),clearing heat and purging fire,and softening hardness and resolving mass.The core prescriptions are the Zhuye Shigao decoction(竹叶石膏汤)and the Siwu decoction(四物汤).
