基于“阳化气,阴成形”理论探讨肿瘤铁过载机制的中医内涵

摘　　要：肿瘤的发生率逐年升高,但目前对其发生机制尚无明确定论。随着铁过载理论被不断探索研究,越来越多的学者认为铁过载可能是肿瘤发生的重要因素。中医学认为,阳主推动、温煦,阴主沉降、成形,阴阳二气各守其道。而铁作为物质形态,属于“阴”的范畴,铁的代谢过程,包括吸收、转运、利用和排泄等,则体现了阳气的推动作用。铁过载作为铁代谢异常的一种表现,不仅直接促进肿瘤形成,还与中医理论中“阳化气不足,阴成形太过”导致“阴积”的概念相吻合,共同加速肿瘤的进展。基于铁代谢与“阳化气,阴成形”的一致性,刘丽坤教授提出治瘤首先应通补阳气,阳气旺盛则阴寒无法凝聚,气血运行顺畅,邪气难以侵袭。临床还需结合辛散通达之品,以温化、温散、温消的方式使阳气通达全身,并且根据虚实情况在温阳药物的基础上加用活血通络之品,以助阳气运行。文章基于“阳化气,阴成形”理论探讨肿瘤铁过载机制的中医内涵,分析铁过载与肿瘤发生、进展的相关性,进而探索“铁过载–阳化气,阴成形–肿瘤”的相关机制,为中医理论在肿瘤防治领域的应用提供新的思路和方法。The incidence of tumors has been increasing year by year,but the mechanism of occurrence is currently uncertain.With the continuous exploration and research of iron overload theory,more and more scholars believe that iron overload may be an important factor in the occurrence of tumors.Traditional Chinese medicine believes that Yangqi(阳气)has the function of promoting Qi(气)and blood circulation and can warm the body,while Yin(阴)-fluid has the function of descending and can nourish the body,both of them have their own roles to play.As a material form,iron belongs to the category of Yin.The metabolic process of iron,including absorption,transport,utilization,and excretion,reflects the driving effect of Yangqi.Iron overload,as a manifestation of abnormal iron metabolism,not only directly promotes tumor formation,but also aligns with the concept of accumulation of Yin-fluid caused by the theory of insufficient Yangqi,and excessive formation in traditional Chinese medicine,jointly accelerating the development of tumors.Based on the consistency between iron metabolism and the theory of“Yang transforming Qi,Yin shaping up body”,Professor LIU Likun proposed that in the treatment of tumors,the first step should be to supplement Yangqi.If Yangqi is strong,Yin-cold cannot condense,Qi and blood flow smoothly,and evil Qi is difficult to invade.In clinical practice,it is necessary to combine pungent taste Chinese medicine to promote the circulation of Yangqi throughout the body through warming,dispersing,and dissipating.Depending on the deficiency or excess,in addition to warming Yangqi medicines,blood activating and meridian unblocking products should be added to promote the circulation of Yangqi.This article explores the the mechanism of traditional Chinese medicine connotation of iron overload in tumors based on the theory of“Yang transforming Qi,Yin shaping up body”,analyzes the correlation between iron overload and tumor occurrence and development,and further explores the related mechanism,aiming to provide
