林洪生教授“固本清源”思想指导下辨治肾癌经验探微

摘　　要：现代医学常用的肾癌治疗方法以手术、靶向及免疫治疗为主,但该病对于放化疗敏感性较低,故而寻求有效的中医药治疗以丰富肾癌治疗手段,延长肾癌患者生存期具有非常重要的现实意义。林洪生教授提出的“固本清源”理论是中西医结合分阶段规范化治疗肿瘤的理论体系。“五治”法是“固本清源”理论在恶性肿瘤不同治疗阶段的具体应用。肾癌的发病基础是正气亏虚、脏腑功能紊乱,在此基础上的痰、瘀、毒互结是肾癌发生的核心。文章概括介绍林洪生教授以“固本清源”理论为指导原则治疗肾癌的临证思路,“固本”以补先后天之精为主,主要从调补肾、脾、肝、肺功能入手,纠正逆乱的脏腑功能,使肿瘤失去生长内环境,“清源”以祛痰除瘀解毒为主,消除痰、瘀、毒等致病因子,防止肿瘤复发;同时详细介绍了林洪生教授在肾癌治疗的不同阶段采用的“五治”方法及遣方用药特点,以推动林教授学术思想的传承与推广。The commonly used treatment methods of kidney cancer in modern medicine include surgery,targeted and immunotherapy,but the disease showed low sensitivity for radiotherapy and chemotherapy.Therefore,it is of great practical significance to seek effective TCM treatment to enrich the treatment methods of kidney cancer and prolong the survival period of kidney cancer patients.Professor LIN Hongsheng’s theory of“strengthening the foundation and clearing the source”is the theoretical system of phased and standardized treatment of tumors in the integrative medicine.The“five-treatment”method is the specific application of the theory of“consolidating the root and clearing the source”in different treatment stages of malignant tumors.The basis of kidney cancer is the deficiency of vital Qi(气)and the dysfunction of viscera.On this basis,phlegm,stasis and poison infection is the core of kidney cancer.The article introduces Professor LIN Hongsheng’s theory as the guiding principle,among which“consolidating the root”is based on supplementing innate essence and acquired essence to regulate the functions of kidneys,spleen,liver,and lung to correct the viscera function,make the tumor lose growth environment,“radical”to remove phlegm,blood stasis,poison and other pathogenic factors to prevent tumor recurrence,and the“five-treatment”method in different stages of kidney cancer treatment is introduced to promote the inheritance and promotion of Professor LIN’s academic ideas.
