基于数据挖掘的穴位按摩治疗卒中后吞咽障碍的选穴规律研究

摘　　要：目的:运用数据挖掘技术探讨穴位按摩治疗卒中后吞咽障碍的选穴规律。方法:检索PubMed、Embase、Cochrane Library、Web of Science、万方数据库、中国生物医学文献数据库、中国知网、维普等各大数据库建库至2024年5月有关穴位按摩治疗卒中后吞咽障碍的文献,建立数据库,使用Excel对选穴频次进行统计,SPSS Modeler 18.0、SPSS Statistics 26.0软件进行关联规则分析和聚类分析。结果:共纳入91篇文献,涉及腧穴处方91首、腧穴44个,腧穴使用总频次为494次,使用频次居前3位的腧穴为廉泉穴、风池穴、翳风穴。关联规则分析结果显示有5组强关联规则穴组,核心穴组为“廉泉穴–风池穴”。使用频率最高的经脉为任脉,腧穴选用部位以颈部为主。结论:穴位按摩治疗卒中后吞咽障碍取穴规律以“经脉所过,主治所及”为主,并根据临床辨证灵活加减腧穴组合,以利咽开窍、疏风通络,从而达到治疗效果。Objective:The acupoint selection rule of acupressure therapy on post-stroke dysphagia was explored by data mining technology in this study.Methods:The published literature on treating post-stroke dysphagia by acupressure therapy was retrieved in PubMed,Embase,Cochrane Library,Web of Science,Wanfang Data,CBM,CNKI,VIP and other databases from the establishment of each database to May 2024 to establish a database.The frequencies of acupressure points were calculated by Excel,and the association rule analysis and cluster analysis were conducted by SPSS Modeler 18.0 and SPSS Statistics 26.0 software.Results:A total of 91 articles were obtained,involving 91 acupoint prescriptions and 44 acupoints.The total usage frequency of acupoints was 494 times,and the top 3 acupoints were Lianquan(RN23),Fengchi(GB20),and Yifeng(SJ17).The results of the association rule analysis showed 5 groups of strong associated acupoints,and the core acupoint group was Lianquan-Fengchi.The most frequently used meridian was the Ren meridian(任脉),and the selected acupoints were mainly located in the neck.Conclusion:Acupoint selection rule of acupressure therapy in the treatment of post-stroke dysphagia is based on the principle of stimulation of meridians can treat diseases of organs corresponding to meridians,and acupoint combinations can be flexibly added or subtracted according to the clinical diagnosis to benefit pharynx and open orifices,and dredge winds and clear the collaterals,so as to achieve the therapeutic effect.
