显微镜下精索静脉低位结扎术联合养精种子丸对不育症患者的影响

摘　　要：目的:观察显微镜下精索静脉低位结扎术联合养精种子丸对不育症患者的影响,探讨其作用机制。方法:选取2022年3月-2023年9月在北京中医药大学深圳医院泌尿外科住院的60例精索静脉曲张伴不育症患者作为研究对象,按治疗方式将其分为观察组和对照组,每组30例。观察组行显微镜下精索静脉低位结扎手术,同时口服养精种子丸治疗。对照组只行显微镜下精索静脉低位结扎术,之后静止观察。12周后,比较两组患者精液参数、睾丸动脉血流参数、相关性激素水平。结果:治疗后,观察组精子密度、向前精子运动率、精子活力以及正常精子率高于对照组,精子畸形率低于对照组(P<0.05)。治疗后,观察组睾丸包膜动脉的收缩期峰值流速(Peak Systolic Flow Velocity,PSV)、搏动指数(Pulsatility Index,PI)、阻力指数(Resistive Index,RI)低于对照组(P<0.05)。治疗后,观察组激素睾酮水平较对照组升高,而促卵泡激素(Follicle–stimulating Hormone,FSH)、促黄体生成素(Luteinizing Hormone,LH)水平较对照组下降,观察组改善程度显著优于对照组(P<0.05)。结论:显微镜下精索静脉低位结扎术后采用养精种子丸能提高不育症患者的精液质量,降低睾丸包膜动脉血流阻力,促使睾酮的含量增加。Objective:To observe the influence of microscopic low-level ligation of spermatic vein plus Yangjing Zhongzi Wan(养精种子丸)on infertility,and to explore its action mechanism.Methods:Sixty patients with varicocele accompanied by infertility who were hospitalized in the Department of Urology of Shenzhen Hospital of Beijing University of Traditional Chinese Medicine from March 2022 to September 2023 were selected as study subjects,and were divided into the observation group and the control group according to treatment methods,with 30 cases in each group.The observation group received microscopic low-level ligation of spermatic vein plus oral Yangjing Zhongzi Wan,while the control group took only microscopic low-level ligation of permatic vein,followed by sedentary observation.After 12 weeks of continuous administration of Yangjing Zhongzi Wan,semen parameters,testicular arterial blood flow parameters,and related sex hormone levels were compared between the two groups.Results:After treatment,the sperm density,forward sperm motility rate,sperm vitality,and normal sperm rate in the observation group were higher than those in the control group,and the sperm abnormality rate was lower than that in the control group(P<0.05).After treatment,the peak systolic flow velocity,pulsatility index,and resistance index of testicular peripheral arteries in the observation group were lower than those in the control group(P<0.05).After treatment,the level of testosterone in the observation group was higher than that in the control group,while the levels of FSH and LH were lower than the control group,and the amelioration in the observation group was significantly better than the control group(P<0.05).Conclusion:Yangjing Zhongzi Wan after microscopic low-level ligation of spermatic vein can improve the infertile patients’semen quality,reduce the blood flow resistance in testicular capsular artery,and increase the content of testosterone.
