玻璃体腔注射康柏西普治疗不同OCT分型糖尿病性黄斑水肿的疗效分析

摘　　要：目的观察不同光学相干层析成像技术(OCT)分型糖尿病性黄斑水肿(DME)患者实施玻璃体腔注射康柏西普治疗的效果。方法60例DME患者,根据OCT黄斑水肿形态不同分为视网膜弥漫性增厚(DRT)组、黄斑囊样水肿(CME)组、浆液性视网膜脱离(SRD)组,每组20例。三组均予以康柏西普玻璃体腔注射治疗。对比三组手术前后的最佳矫正视力(BCVA)、黄斑中心凹视网膜厚度(CMT)。结果CME组术前及术后1周、1个月、3个月的BCVA分别为(0.591±0.110)、(0.453±0.121)、(0.476±0.134)、(0.489±0.131),CMT分别为(561.601±100.350)、(408.908±100.369)、(372.567±100.392)、(399.760±100.094)μm;SRD组术前及术后1周、1个月、3个月的BCVA分别为(0.381±0.113)、(0.210±0.130)、(0.198±0.150)、(0.211±0.124),CMT分别为(446.883±100.387)、(347.315±100.408)、(328.896±100.433)、(345.068±100.036)μm;DRT组术前及术后1周、1个月、3个月的BCVA分别为(0.651±0.158)、(0.514±0.132)、(0.521±0.185)、(0.548±0.148),CMT分别为(647.333±168.063)、(454.486±100.066)、(468.322±100.070)、(506.098±100.153)μm。三组组间术前及术后1周、1个月、3个月的BCVA对比,差异有显著性(P<0.05);三组术后1周、1个月、3个月的BCVA与术前对比,差异有显著性(P<0.05)。组间两两对比:CME组与SRD组、SRD组与DRT组术前及术后1周、1个月、3个月的BCVA对比,差异有显著性(P<0.05);CME组与DRT组术前及术后1周、1个月、3个月的BCVA对比,差异无显著性(P>0.05)。三组术前及术后1周、1个月、3个月的CMT对比,差异有显著性(P<0.05);三组术后1周、1个月、3个月的CMT与术前对比,差异有显著性(P<0.05)。组间两两对比:DRT组与CME组术前的CMT对比,差异有显著性(P<0.05);DRT组与SRD组术后1、3个月的CMT对比,差异有显著性(P<0.05);CME组与SRD组术前及术后1周、1个月、3个月的CMT对比,差异有显著性(P<0.05);DRT组与CME组术后1周、1个月、3个月的CMT对比,差异无显著性Objective To observe the therapeutic effect of intravitreal injection of Conbercept on different optical coherence tomography(OCT)subtypes of diabetic macular edema(DME).Methods A total of 60 DME patients were divided into diffuse retinal thickening(DRT)group,cystoid macular edema(CME)group and serous retinal detachment(SRD)group according to the different OCT morphologies of macular edema,with 20 patients in each group.All the three groups were treated with intravitreal injection of Conbercept.The best corrected visual acuity(BCVA)and central macular thickness(CMT)before and after surgery were compared among the three groups.Results In CME group,the BCVA before surgery and at 1 week,1 month and 3 months after surgery were(0.591±0.110),(0.453±0.121),(0.476±0.134)and(0.489±0.131),and the CMT were(561.601±100.350),(408.908±100.369),(372.567±100.392)and(399.760±100.094)μm.In SRD group,the BCVA before surgery and at 1 week,1 month and 3 months after surgery were(0.381±0.113),(0.210±0.130),(0.198±0.150)and(0.211±0.124),and the CMT were(446.883±100.387),(347.315±100.408),(328.896±100.433)and(345.068±100.036)μm.In DRT group,the BCVA before surgery and at 1 week,1 month and 3 months after surgery were(0.651±0.158),(0.514±0.132),(0.521±0.185)and(0.548±0.148),and CMT were(647.333±168.063),(454.486±100.066),(468.322±100.070)and(506.098±100.153)μm.Comparison of BCVA before surgery and at 1 week,1 month and 3 months after surgery among the three groups,the difference was significant(P<0.05).The BCVA of the three groups at 1 week,1 month and 3 months after surgery was significantly different from that before surgery(P<0.05).There were significant differences in BCVA between CME group and SRD group,SRD group and DRT group before surgery and at 1 week,1 month and 3 months after surgery(P<0.05).There was no significant difference in BCVA between CME group and DRT group before surgery and at 1 week,1 month,and 3 months after surgery(P>0.05).Comparison of CMT before surgery and at 1 week,1 month and 3 mon
