多西环素治疗宫腔镜术后子宫内膜炎的效果观察

摘　　要：目的研究多西环素治疗宫腔镜术后子宫内膜炎的效果。方法以100例宫腔镜术后子宫内膜炎患者为研究对象,以随机抽样法分为对照组和观察组,每组50例。对照组患者服用甲硝唑治疗,观察组患者服用多西环素治疗。比较两组患者临床疗效、预后以及治疗前后白细胞介素(IL)-2、肿瘤坏死因子-α(TNF-α)、基质金属蛋白酶-9(MMP-9)、IL-1β水平。结果观察组治疗总有效率98.00%高于对照组的86.00%,有统计学差异(χ^(2)=4.891,P=0.027<0.05)。观察组妊娠率为68.00%(34/50),高于对照组的48.00%(24/50),有统计学差异(χ^(2)=4.105,P=0.043<0.05)。治疗后,两组的IL-2水平较治疗前明显升高,而TNF-α、MMP-9、IL-1β水平较治疗前明显降低,且观察组患者IL-2高于对照组,TNF-α、MMP-9、IL-1β低于对照组,有统计学差异(P<0.05)。结论宫腔镜术后子宫内膜炎选用多西环素治疗疗效确切,不仅可以改善患者的炎症状况,还可以增加后期的妊娠率。Objective To study the effect of doxycycline in the treatment of endometritis after hysteroscopy surgery.Methods 100 patients with endometritis after hysteroscopy surgery were divided into control group and observation group by random sampling method,with 50 cases in each group.The control group received metronidazole treatment,and the observation group received doxycycline treatment.The clinical efficacy,prognosis,and the levels of interleukin(IL-2),tumor necrosis factor-α(TNF-α),matrix metalloproteinase-9(MMP-9)and IL-1β before and after treatment were compared between the two groups.Results The observation group had higher total effective rate of 98.00%than 86.00%in the control group,and there was statistical difference(χ^(2)=4.891,P=0.027<0.05).The pregnancy rate in the observation group was 68.00%(34/50),which was higher than 48.00%(24/50)in the control group,and there was statistical difference(χ^(2)=4.105;P=0.043<0.05).After treatment,the level of IL-2 in both groups was significantly increased compared with before treatment,while the levels of TNF-α,MMP-9 and IL-1β were significantly decreased compared with before treatment;the observation group had higher IL-2 than that in the control group;the observation group had TNF-α,MMP-9 and IL-1β was lower than the control group;there was statistical difference(P<0.05).Conclusion Application of doxycycline shows definite effect for the treatment of endometritis after hysteroscopic surgery,which can not only improve the inflammatory condition of the patients,but also to increase the pregnancy rate at a later stage.
