门冬胰岛素与地特胰岛素联合用于妊娠期糖尿病治疗中的疗效分析

摘　　要：目的探讨门冬胰岛素与地特胰岛素联合用于妊娠期糖尿病治疗中的疗效。方法选取60例妊娠期糖尿病孕妇,随机分为研究组和对照组,各30例。对照组接受门冬胰岛素治疗,研究组接受门冬胰岛素联合地特胰岛素治疗。观察比较两组不良妊娠结局发生情况、血糖指标[餐后2 h血糖(2h PG)、空腹血糖(FPG)、糖化血红蛋白(HbA1c)]、血糖指标恢复正常时间。结果与对照组的20.0%比,研究组不良妊娠结局发生率3.3%更低(P<0.05)。治疗前,两组2h PG、FPG及HbA1c比较并未有显著差异性(P>0.05);治疗后,与对照组的(7.3±1.1)mmol/L、(5.3±1.1)mmol/L、(7.6±0.9)%比,研究组2h PG(6.2±0.8)mmol/L、FPG(4.1±0.8)mmol/L及HbA1c(6.1±0.5)%均更低(P<0.05)。与对照组的(6.5±1.7)、(5.8±1.3)、(8.2±2.1)d比,研究组2h PG恢复正常时间(5.3±0.9)d、FPG恢复正常时间(4.6±1.1)d及双指标恢复正常时间(6.7±1.9)d均更短(P<0.05)。结论妊娠期糖尿病孕妇接受门冬胰岛素联合地特胰岛素治疗能有效增强疗效,可对孕妇血糖水平进行有效控制,使血糖恢复正常时间明显缩减,并促进孕妇妊娠结局改善,可推广使用。Objective To explore the therapeutic effect of insulin aspart combined with insulin detemir in the treatment of gestational diabetes mellitus.Methods A total of 60 patients with gestational diabetes mellitus were randomly divided into the study group and the control group,each with 30 cases.The control group was treated with insulin aspart,and the study group was treated with insulin aspart combined with insulin detemir.The adverse pregnancy outcome,blood glucose index[2 h postprandial glucose(2 h PG),fasting plasma glucose(FPG),glycated hemoglobin(HbA1c)]and the recovery time of blood glucose index were observed and compared between the two groups.Results Compared with 20.0% in the control group,the incidence of adverse pregnancy outcomes was lower in the study group at 3.3%(P<0.05).Before treatment,there was no significant difference in 2 h PG,FPG and HbA1c between the two groups(P>0.05).After treatment,2 h PG(6.2±0.8)mmol/L,FPG(4.1±0.8)mmol/L and HbA1c(6.1±0.5)% were lower in the study group compared to(7.3±1.1)mmol/L,(5.3±1.1)mmol/L and(7.6±0.9)% in the control group(P<0.05).The recovery time of 2 h PG,FPG and both indexes were(5.3±0.9),(4.6±1.1)and(6.7±1.9)d in the study group,which were shorter than(6.5±1.7),(5.8±1.3)and(8.2±2.1)d in the control group(P<0.05).Conclusion The treatment of insulin aspart combined with insulin detemir can effectively enhance the therapeutic effect,effectively control the blood glucose level of patients,significantly reduce the recovery time of blood glucose,and promote the improvement of the pregnancy outcome of patients,so it can be widely used.
