基于药品属性的预溶量自定义策略在儿科静脉药物配置中心的实施效果评价

摘　　要：目的对于根据药品属性自定义预溶量在儿科静脉药物配置中心(PIVAS)实施的效果进行评价,以提高药物配置的安全性和效率。方法按照药品属性将药品分类,设计问卷调研,根据调研结果自定义药品预溶量,以实施自定义预溶量策略前后半年调配差错率、调配时长、每批次出科准点率、人力成本、调配药师满意度为评价指标。结果输液标签上增加药品预溶量,利用计算机终端计算医嘱药品按预溶量预溶后需要调配的终剂量,调配人员只需按标准操作即可,根据药品属性自定义预溶量,调配差错率由0.11‰减少到0.02‰,平均每袋成品输液调配时间由(1.20±0.30)min缩短到(0.91±0.21)min,出科准点率由99.23%提升到100.00%,调配人员由48人减少到44人,调配药师满意度提升。结论根据药品属性制定的自定义预溶量,减少药品配置过程的剂量换算,不仅能减少人工计算的差错和时间,提高患者用药安全,而且能节约人力成本。Objective To customize the pre-dissolution dose based on the attributes of the drug is implemented in pharmacy intravenous admixture services(PIVAS)to improve the safety and efficiency of the drug configuration.Methods Classify drugs according to their properties,design questionnaire survey,customize the pre-dissolution amount of the drug according to the survey results,the evaluation indexes were the dispensing error rate of half a year before and after the customized pre-dissolution amount,dispensing time,on-time rate of each batch,labor cost and dispensing pharmacist satisfaction.Results The infusion label has added the pre-dissolution amount of the drug,the computer terminal is used to calculate the final dose of the prescribed drug after pre-dissolution,the dispensing personnel only needs to follow the standard operation.According to the self-defined pre-dissolution volume of the drug properties,the dispensing error rate decreased from 0.11‰to 0.02‰,average infusion deplagment time per bag of finished product shortened from(1.20±0.30)min to(0.91±0.21)min,the on-time delivery rate increased from 99.23%to 100.00%,and the dispensing personnel decreased from 48 to 44.The satisfaction of dispensing pharmacists was improved.Conclusion According to the properties of the drug,the customized pre-dissolution volume can reduce the dose conversion in the drug configuration process,which can not only reduce the error and time of manual calculation,improve the drug safety of patients,but also save labor costs.
